GET YOUR WELLNESS INCENTIVE LOWER YOUR MEDICAL PLAN DEDUCTIBLE If enrolling in the medical plan, you and your covered spouse must do the following to receive the reduced medical plan deductible: Complete a Personal Wellness Profile (PWP) Attend a Biometric Screening* Meet or achieve tobacco-free status by: screening negative for tobacco use and if you are a tobacco user, designating on the PWP that you Commit to Quit in 2012. You and your covered spouse must complete the steps to reduce your medical plan deductible within 60 days of your hire date for new employees and family status changes (date of marriage, loss of other coverage, etc). FTE status changes have 60 days from the date the change was entered into the Hospital Information System PWP and Biometric Screening Instructions Step 1 Complete a Personal Wellness Profile (PWP Go to mymethodistbenefits.com Scroll down and click on Enter your User Name and Password. Employees: Enter your TMHS Network ID for both the User Name and Password (IN ALL CAPS). Example: User Name = TMHXYZ Password = TMHXYZ Spouses: Enter the employee’s Social Security Number plus the word “SPOUSE” for both the User Name and Password (IN ALL CAPS). Example: User Name = 123456789SPOUSE Change your password and setup your security question (make note of your new password). Click on Personal Wellness Profile™, located on the Main Menu on the left-hand side of the screen. Review the Consent and Notice to Users and select Agree or Disagree. If you disagree to the terms, you will not be allowed to complete the PWP. Click on Start New Assessment and complete the questionnaire. Make sure you click Finish Questionnaire and Save and Exit upon completion. You will be able to print your PWP once your Biometric data has been entered. Step 2 Complete a Biometric Screening Schedule an appointment with Methodist Wellness Services to complete your biometric screening (height, weight, waist circumference, lipid profile, blood pressure, glucose and tobacco testing). See the table below for available dates, times, and contact information. Employees and spouses can make an appointment at any Methodist location that is convenient for them, regardless of the employee’s work location. Spouses who reside more than 50 miles from a Methodist facility can visit any physician and have them complete a Methodist Offsite Biometric Form. To request a form, contact Methodist Wellness at 713-441-5936. Results must include height, weight, blood pressure, waist girth, cotinine testing, total cholesterol, LDL, HDL, triglycerides and glucose. Employees are not allowed to use the form.
