Note: This form MUST be received by Hoopz Management before the first game of the season.

The undersigned hereby waives any and all claims for injuries, damages, losses, costs, liabilities or
expenses to player’s personal wellbeing and/or properties, of any nature or kind, which may be caused
by any act, or failure to act, of the League, the officials and/or employees. Please read below
acknowledgements carefully and signed at the bottom once you have fully understood the information:





The personal information collected here will be solely used for league administration purposes
as per Hoopz Basketball League privacy policy.
To ensure that only registered players participate in any of the Hoopz Basketball games, proof of
a photo ID may be required upon request.
Hoopz Basketball League’s regulations contain disciplinary procedures including fines and
suspensions that team members may be subject to if they violate the rules contained therein.

Due to the nature and levels of Hoopz Basketball, player must recognize and be aware of the
fact that serious injuries may occur in the facilities rented and/or during the duration of the
game as a result of participating in the league
Any injuries, damages, losses, costs, liabilities or expenses will be at the individuals or teams
discretion. Hoopz Basketball will not be held liable for any of the above stated.
