Clan Macnab DNA Project Participant’s Consent

Hereby give my consent to the Clan Macnab DNA Project Administrators and/or their
successors and assigns as follows:
1) For my DNA results to be used for research within the Clan, and for my results to
be posted on Clan websites and in Clan newsletters: (check one)
 With my name, cadet house association and oldest known and speculative
ancestors noted
 With my cadet house association and oldest known and speculative
ancestors noted, but not my name.
2) For my results to be posted and/or discussed publicly on other websites, forums,
or discussion groups, for the purpose of furthering research and understanding of

 With my name, cadet house association and oldest known and speculative
ancestors noted
 With my cadet house association and oldest known and speculative
ancestors noted, but not my name.
 for the results to be disseminated to research groups outside the clan, without
names attached, but with oldest known ancestor and cadet affiliation noted.

 I agree that in the case of my death, I appoint The Clan Macnab Trust as the
beneficiary of any stored DNA samples and expressly give them permission to
undertake further testing as deemed appropriate and in the best interests of the
Clan at large. (check)
I acknowledge that as this is an International Research Project, I explicitly waive any
rights I may have under any Privacy or Freedom of Information Act in my country of
residence, and instead give my specific consent as outlined above.
I agree to hold harmless and indemnify the volunteer administrators of this project from
any damages arising from the use of my personal information as specified.
