CHAPTER CHAIR CHECKLIST Updated: 4 February 2008 Ontario Branch Ontario Branch Chapter Chair Checklist Upon Election to Position: 1. Identify your presidential liaison; they will provide you with an orientation package. This officer will be representing the internal portfolio. 2. An e-mail, from the outgoing Chapter Chair, should be sent to chapter members to introduce the new, incoming Chapter Chair. 3. Obtain the most recent list of members in your chapter from the outgoing Chapter Chair. The National Office will provide the membership list automatically three (3) times a year on or about March 1 st, August 1st and November 1st. This list will include e-mail addresses, which may be the most convenient way to communicate with your membership base. Currently, the representative from national who supports membership lists is Robin Rockwell ([email protected]). 4. Create an e-mail distribution list for sending out invitations to CE events and newsletters to the chapter membership. When sending out information, please refer to the privacy requirements outlined below: Email and Privacy The CSHP Privacy Policy aims to protect the privacy of its members, including e-mail addresses. As Chapter Chair you will have access to e-mail addresses for your chapter members. E-mails to chapter members should be sent as a “blind copy” for privacy purposes. Only CSHP officers (Executive, Chapter Chairs, administrative support, task force/ committee chairs) should have access to member e-mail addresses. E-mails to members concerning industry sponsored symposiums and events at CSHP Conferences will be sent by CSHP officers, not individual event sponsors. At no time can the e-mail addresses of members be used for purpose of personal or corporate business. Your name and e-mail address will be posted on the CSHP website unless you expressly decline. 5. Plan a meeting to introduce yourself personally to your chapter members. Ideally this would be in person and perhaps in conjunction with an upcoming CE event. If there are no standing plans for an upcoming CE, a meeting can likely be arranged in a location central to your chapter with minimum cost (e.g. using a hospital conference room over a lunch hour or after work). This meeting can be used to introduce yourself, get acquainted with your members, and find out their focus for education over the coming term and their views on some of the key issues you would have been introduced to at the Ontario Branch Annual General Meeting. Ontario Branch, CSHP 4 September 2007 2 6. Alternatively a newsletter can be created and sent electronically to your chapter to accomplish what you would have in a face-to-face meeting. Using Microsoft Word (or other word processing software) is usually the most efficient way to accomplish this task. The finished product can be e-mailed to members as an attachment and also posted on the Ontario Branch website for reference. Please contact the OB webmaster for help in posting documents. For posting on the website, follow these steps: Select a ‘SECTION’ to post in, and click. On the top bar, click ‘CONTRIBUTE’, then type in a ‘TITLE’ for the post. Type in a short ‘INTRO’. Type in the body of the message, and click ‘SAVE’. This also helps introduce members to the website and increase the website’s visibility and usefulness. See attached example of a newsletter used at the beginning of a Chapter Chair’s term (Appendix I). 7. Hold an election for Chapter Executive. Positions may include VP, Secretary and Treasurer. A call for nominations can be sent out via e-mail, setting a deadline for people to respond. Ideally the election could be held as a short business meeting. A vote, if needed, can be a simple show of hands, mailed out ballots or can be conducted among membership by e-mail if necessary. Nominees should be provided with the opportunity to present their candidacy. The type of presentation will depend on the election format (i.e. written or verbal presentation). Inform chapter members of election results in a timely manner. 8. Set a meeting with Chapter Executive to establish goals for CE events, delegate tasks to organize the events, and to determine frequency of committee meetings throughout the year. Future meetings will be needed to discuss upcoming CE events, Residents Poster Presentation Night (if applicable), newsletters and contributions to HPO. Refer to the continuing education guidelines and checklists posted on the website. Check the Chapter Chair section on the OB website (www.cshpontario.ca) 9. As part of each chapter’s mandate, CE events are to be provided for the membership. The number of CE events will depend on the chapter and the availability of sponsors and speakers, but a goal of 4-6 CE events per year is suggested. The CE forum also serves as a way to communicate information from CSHP OB to the membership Organizing a CE event: Dates for CE events should be selected so that they do not interfere with other CSHP events or ideally with other local pharmacy events. Check the CSHP, OCP and OPA websites for a listing of events by the respective organizations. Consult neighboring chapters and local OCP CE Coordinators (www.ocpinfo.ca for contact information) to coordinate and collaborate on CE events and topics. Topics for CE events can be selected based on feedback from previous events from the membership (see Appendix II, CE Feedback Form) or based on consensus from the Chapter Executive. Topics should be broad enough so that the information presented will be relevant to the vast majority of the membership. Ontario Branch, CSHP 4 September 2007 3 Speakers for CE events may be selected by the chapter or by the sponsor for the event. If being selected by the chapter, this is usually done prior to obtaining funding from a sponsor so that speaker’s name can be included when requesting funding for the event. Consider asking speakers to permit posting of their presentation on the CSHP-OB website. If so, there is a release for them to sign (see Appendix III) and the electronic file should be forwarded to the webmaster after the presentation. Chapter Chairs should ask speakers to disclose any conflict of interest prior to their start of their presentation. After a topic is decided upon, a list of potential sponsors should be generated: Sponsor contact information can be obtained by contacting the company and asking for the local representative, or may be solicited from colleagues who already have an established relationship with the local representative for the company in consideration. The expectation of the sponsor is that they will provide funding to host the CE event including an honorarium for the speaker, and if alcohol will be served, the cost of obtaining event insurance through CSHP national office (see below.) The sponsor generally makes all the arrangements for the CE event including picking the forum, organizing the reception, negotiating an honorarium with the speaker and organizing the speaker’s AV requirements. The sponsor should be advised, in advance, that CSHP National offers annual sponsor recognition to pharmaceutical companies for their support each year. Should they wish to receive recognition for their company’s sponsorship towards a CE Event, the company name and amount of sponsorship will be forwarded to National and will be listed in the final programs for both PPC and the National AGM. Invitations for the event are sent out by the chapter (see Appendix IV Sample Invitation.) To eliminate the cost of mailing invitations, e-mail should be utilized as much as possible. (Refer to the e-mail policy, above). The invitation should also be sent to other Chapter Chairs for information sharing about potential speakers and topics, as well as to the Administrative Assistant for corporate sponsorship information. The invitation can also be posted on the CSHP OB website. Please see Appendix VI regarding CSHP’s Branding Policy which should be taken into consideration when sending out any CSHP communication including invitations to CE events. The chapter also deals with RSVPs for the event. Please note that the number of attendants to any given event may be limited by the amount of funding provided by the sponsor. If this is the case then it should be clearly communicated on the invitation that priority will be given to CSHP members. If there are still spots open after the RSVP deadline, then these spots may be offered to non-CSHP members. Non-members are charged up to$20 to attend chapter CE events except for Resident’s Poster Event. This fee can be applied to a CSHP membership for the current or upcoming year. The names of non-members who have paid to attend a CSHP event should be sent after each event by email to Membership Administrator at CSHP National Office. After all RSVPs have been received a sign-in list of attendees should be generated for the event. This list can be given to the industry representative if requested. The chapter introduces the speaker; therefore a short biography/CV should be obtained from the speaker in advance of the event. Information from CSHP OB to be communicated to the members at the event should also be prepared. CSHP chapter CE events can obtain approval from OCP for CE credits. If applying for approval, you will need three (3) learning objectives for the program and will need to submit a form to OCP (see Appendix V). This should be done as far in advance as possible to allow OCP to advertise the event on their website or in Pharmacy Connections if desired. Ontario Branch, CSHP 4 September 2007 4 Event Insurance: Liability insurance is required and provided to Ontario Branch through CSHP’s National comprehensive general liability insurance. This insurance covers all events held by Ontario Branch including Chapter CE Events. All events, including those involving alcohol will be automatically covered within the policy year (June 1st to May 31st). Unless otherwise notified by the CSHP National Office, this policy will remain in effect and be renewed annually. The Ontario Branch is responsible for paying an annual fee towards this coverage; it will cover all events for Ontario Branch in the course of the policy year. In all cases where event insurance is required, CSHP national requires the following information two (2) weeks in advance of an event taking place: location, including full address, date of event, start time, number of expected attendees, and a brief description of the purpose/nature of the activity. This information should be forwarded to the Ontario Branch Administrative Assistant who, in turn, will forward it to the appropriate person at the National Office. 10. Book time off work for the Ontario Branch Mid-Term Council Meeting and the AGM. These dates are available on the CSHP website and can be forwarded by the CSHP OB support staff, ([email protected]). 11. Contact the Presidential Officer for the Internal Portfolio with any concerns or issues in your chapter that require immediate attention/response. There will be a teleconference for all Chapter Chairs and the presidential liaison officer on the second Tuesday of every 2nd month from 4-5 p.m. 12. Expect to receive Executive Council Meeting Minutes each month by e-mail, once they are approved. 13. Expect to receive e-mail updates from National Council Meetings from the Ontario Branch, National Delegates. Contact one of the National Delegates on council at any time for national issues. Two Months Before Council Meetings: Prepare a report for Council. Chapter Chair Procedure for Council Reports Council reports are submitted one month before the Ontario Branch Annual General Meeting (AGM) and Mid-Term Council (MCM) meetings. All Chapter members receive a draft copy of the Council reports prior to submission for comments and feedback and a final copy upon submission to the administrative assistant. The OB administrative assistant emails the Chairs the report template and guidelines one month before the report due date (see attached Appendix VII). The style of the report is point-form. The length of the report is 1 to 2 pages. The main purpose of the report is to: a) update Council on completed and planned activities and achievements over the past six months, and b) submit items for consideration or recommendations on behalf of the Chapter Chapter activities and achievements should coincide to the Branch strategic plan where possible. Ontario Branch, CSHP 4 September 2007 5 During the process of creating a report, there may be items that merit further discussion or require a decision that is not strategic in nature. In these circumstances, CSHP – OB members should identify such issues in the “Items for Consideration” (IFC) section. An IFC does not require special formatting or content. It is important that enough supporting information is provided so the IFC can be explored further. All Recommendations and Items for Consideration submitted in reports will be reviewed by the Council Executive prior to Issues Day. They will be organized into the agenda such that similar topics are discussed together and all recommendations and IFC are addressed. It is critical that all reports be submitted by the deadline to facilitate the creation of the Issues Day Agenda Recommendations are strategic in nature and are for discussion or debate by Council at Issues Day and/or the business meeting. The CSHP guidelines for making recommendations are followed (Council and Branch Reference Manual, Section 8). Within One Month after Meetings: Submit Expenses: Submit expense report for reimbursement (to Ontario Branch, Administrative Assistant). Complete Chapter ‘Write Up’ and/or ‘Report’: Determine when you will need to submit a section for HPO. Periodically Chapter Chairs will be asked to submit items to the Communications Chair to be published in the “HPO”. There are two types of publications that can be requested: 1) Chapter Chair Corner and 2) Reports and Post Council Updates. Chapter Chair Corner submissions can include anything of interest to the membership regarding the chapter. It could include a profile of the chapter or members within the chapter or a summary of events or occurrences that impact hospital pharmacy. The target audience is CSHP members in Ontario and topics need to be relevant to this audience. The write up should be no longer than 2 pages double-spaced and may include a picture. Lengthier articles may be accepted depending on space and discretion of the HPO editor (it is best to check with them first). During your term as chapter chair you will generally be asked to submit this once. After a council meeting you may be asked to submit a report or update from the meeting. This is generally assigned during the meeting and should be submitted within one month of the meeting whenever possible. By posting the presentation to the website we hope to increase the knowledge base of Pharmacists by allowing them access to educational material that they may not have been able to attend in person. What is a web based presentation? A web based presentation takes the live presentation and c0nverts the material into a self directed learning module. The slides and voice track will be used to guide the participant through the exact presentation. What are the components that are being posted to the web? The web will contain a copy of the slides you used to create the original presentation and an audio recording of the presenter’s narration. Additional material may be included in the web based presentation such as presenter’s biography, venue information, self assessment questions, references and other appropriate material created for the presentation. Are my slides protected from copying? Your slides will be protected from electronic transmission and copying use a PDF format. Who will be able to access my presentation? It cannot be altered or distorted optically in any way. 2- The CSHP logo should be predominantly displayed by all CSHP related bodies such as branches, task forces, committees, PSNs etc…on official publications, letter head, web site or any other visual display where a logo is used. 3- No person, without the authority of the Executive Committee, shall assume or use the CSHP Corporate Logo, device or design so nearly resembling the CSHP logo as to be likely to be confused therewith on any letter, pamphlet, notice or handbill. 4- Persons authorised to use the Corporate Logo on Society business are the Executive Officers as well as those designated by the executive to hold this authority. The Corporate Logo is not to be used by any member of the Council or other person for purposes that are not in line with the corporate vision of the Canadian Society of Hospital Pharmacists. 5- The development of new logo by all CSHP bodies should be pre-approved by the Executive director. OBJECTIVE To provide for the protection of the use of the CSHP Corporate Logo and to prohibit its use for unofficial purposes. To promote its use in all the activities of the Society. RATIONALE Identity plays as important a strategic role in meeting our goals, as do any of our organizational and business elements. A strong corporate identity helps strengthen our image and support our marketing activities. When our corporate identity is presented consistently, we create a positive and lasting impression of our association. The value of our brand symbol (logo and corporate identity) is immeasurable. Its value increases each time it is presented properly and decreases with every incorrect or improper application. This branding policy will assist us in ensuring the proper use of the Canadian Society of Hospital Pharmacists (CSHP) trademarks. Our trademarks are important and valuable assets because they represent the standards of quality and innovative thinking associated with the Society. PROPER USE OF THE CSHP LOGO CSHP logo is the common and unique graphic thread that binds the Society together. The simple rule is that the logo must be readily seen and positioned so its impact is not diluted by other words or visual elements. It cannot be altered or distorted optically in any way. The logo is to be used with either the name in full or abbreviated in both in English and French. All branches must predominantly use the CSHP Logo in concert with Branch logos. The branch logo should not appear as a standalone logo. Ontario Branch, CSHP 4 September 2007 15 SOCIETY COLOURS The society colours are PMS (Pantone Matching System) are Blue (286) and Pantone 676 (equivalent of black). These are the only acceptable colours for two-colour reproduction of the logo. When reproducing the logo in colour, the process equivalent (cmyk breakdown) is used. The process equivalent of PMS is: The CYMK for the two PMS colours are: Reflex Blue: CMYK Cyan: 100 Magenta: 60 Y (yellow): 0 K (black): 6 Pantone 676: CMYK Cyan: 6 Magenta: 100 Y (yellow): 0 K (black): 22 Other colours may be used on promotional material (vest, T-shirt, etc… ), in this situation the colour scheme must be approved by the national office to ensure it does not infringe on other well recognised logos (for example the Swiss army knife if red colour is used). The CSHP Corporate Identity Standards are designed for CSHP members, branches as well as CSHP employees and agencies that are authorized to use the corporate logos in creating presentations, packaging, collateral or ads. Please follow this guide carefully to make sure logo usage complies with CSHP corporate policy. Your help and support in applying the guidelines strictly and consistently will add to the strength of our society. Anyone who intends on using any of our trademarks should become familiar with these guidelines. Any questions regarding this policy may be directed to our executive director.
