Welcome to the 2015-2016 school year

On behalf of Coordinated Student Health Services, we
wish you a successful and healthy school year!
Students may require the provision of, or assistance with, medical procedures
while in school. Some examples include: students with diabetes, asthma,
seizures or severe allergic reactions to foods or insect stings.
Some students may require assistance from trained healthcare personnel,
while other students may be able to be assisted by trained school personnel.
A completed Authorization for Medication/Treatment form is required for all
medication/medical procedures. Coordinated Student Health Services will
determine the type of services required to meet the individual student’s needs.
Students receiving medications for Asthma, while in school, are encouraged
to get a School Asthma Action Plan from their Healthcare Provider.
Students with Diabetes will need a Diabetes Medication/Treatment
Authorization form from their Endocrinologist. It is the responsibility of the
parent/guardian to provide all necessary supplies. This includes a meter with
lancets and test strips to check blood glucose, insulin and syringes, glucose gel
and/or glucagon to treat extremely low blood sugar, and all necessary snacks
and juices to treat low blood sugar.
The School Board of Broward County, Florida takes great pride in ensuring
that your child has the very best school experience possible. A student with
medical needs requires a teamwork approach. CSHS provides trained
personnel to help manage your child’s needs during school hours.

lunch

school
