Outcomes-in this section you will need to think about what the long term educational
outcomes are for this child/young person and what are the steps needed to meet the
outcome. These outcomes will form part of the final EHC plan.
What is meant by Long Term Outcome(s):
These are the outcomes sought for the child/young person to succeed and achieve. In
identifying an outcome consider what is important to the child/young person, what they want
to achieve, and what is important for them as judged by the class teacher/SENCO and
others with their best interests at heart.
Outcomes are long term and usually set out what needs to be achieved by the end of the
phase or stage of education in order to enable child/young person to progress to the next
stage of their education. For child/young person who are Year 9 and onwards that should
include preparation for adulthood.
There should be no more than three Long Term Educational Outcomes included.
Outcomes are a description of the benefit or difference made to an individual as a result of
an intervention. Outcomes should be phrased as what the child/young person will achieve.
The outcome will be followed by ‘steps towards the outcome’
What is meant by a Step(s) Towards an Outcome?
It is likely that a broad Outcome will be supplemented by one or more ‘Step(s) towards the
Outcome’, and are shorter term goals likely to be achievable within the next 12 months.
Please see example below.
Long Term Outcome:
