Map & Directions
MPCC is a leader in chronic disease management for diabetes and hypertension. Patients receive free health education and are provided with tools to monitor
their condition at home with items such as glucometers and blood pressure monitors.

MPCC works with pharmaceutical patient assistant programs to enroll uninsured patients for free medication.

Our Special Personal Assistance (SPA) program provides homeless men and women with hot showers, clean clothing, and helps them gain access to housing, substance abuse treatment, and employment training programs.

MPCC need your support? MPCC provides care for the uninsured and those
at the highest risk of experiencing poor health.

To achieve positive patient outcomes, MPCC provides more than just an
exam room. Our medical home model includes a wide variety of services for
patients to ensure that each patient understands their health care needs and
follows through on obtaining the care they need.

Margaret M. Meyers, M.D. Medical Director
Since the age of 15, Dr. Meyers knew that she had a calling to provide
care for the underserved. Dr. Meyers found her home at Mercy Primary Care
Center in January of 2001. Since then she has been considered family by both
her patients and her staff. She is a dedicated physician who genuinely cares for her patients. Dr.
Meyers' patients feel that they are valued and important, and thereby, they are encouraged to
take care of themselves at home when they leave MPCC. Dr. Meyers truly embodies the
mission of MPCC, to improve the quality of life of her patients.
Pamela Williams, M.D. Staff Physician
Dr. Williams has been a part of the Mercy Primary Care Center family since
its beginning in 2001. Dr. Williams is a model of what being a family physician
means. She is knowledgeable and passionate about improving her patients'
health. Moreover, she sets the example of leading a healthy lifestyle. She is using this personal
passion for healthy behaviors to help empower her patients. Her personal contributions to the
clinic have led to improvements in the quality of and access to healthcare. Dr. Williams' compassion is not only seen in her devotion to her career. She also uses her spare time to volunteer
at her children's schools as well as other local elementary schools. She also goes on frequent
medical mission trips to Haiti. Dr. Williams' smile and pleasant demeanor act as a "ray of sunshine" to both her patients and staff. She truly brings joy to MPCC.
Margaret A Wells, A.N.P. Nurse Practitioner
Margaret Wells, ANP brings a wealth of experience (over 15 years!) as a
nurse practitioner in an urban setting. She is a caring, talented provider who
cares for complex patients with knowledge and skill. Her big heart and great dedication to her
patients is inspiring. She is not afraid to get to know her patients at a deep level in hopes of supporting them in their health goals. She brings a great enthusiasm to her work and is clearly committed to outstanding patient care.
Chronic Disease Management
Chronic disease management is the backbone of our practice. More than half of patients receiving care at MPCC have diabetes, hypertension, depression or a combination of all three.
To address this need, we follow a Medical Home model of care, where our patients are engaged as partners in their care.

Diabetes: Over 20% of MPCC patients have diabetes. MPCC offers a coordinated program which has helped patients take control of their condition. On average, MPCC has
been able to reduce Hemoglobin A1c for diabetic patients by at least 1.0 percentage
point– reducing their risk of complications and death by over 20%.

Hypertension: Approximately 44% of MPCC patients have hypertension. By providing
patients with resources such as a home blood pressure monitor, MPCC has helped patients improve blood pressure control which will result in lower rates of heart disease and
stroke.

Depression: Approximately 14% of MPCC patients have been diagnosed with depression. MPCC provides education and medication management while also collaborating
with the University of Detroit Mercy to provide access to a mental health nurse practitioner, who provides talk therapy.

MPCC has been recognized locally and nationally for excellence in chronic disease management:

MPCC has achieved Trinity Health Notable Practice status for Diabetes and Congestive Heart Failure
– reflection that care exceeds national standards for process and health outcomes.

For Feb. 2017 – Feb. 2018, MPCC has been awarded Level 2 Patient-Centered Medical Home Recognition
from the (NCQA) National Committee for Quality Assurance.
Pharmacy
The Pharmacy at Mercy Primary Care is a crucial aspect of Mercy's mission of patient centered care. The onsite pharmacy allows primary care and medication management to be well coordinated. Our providers consult with the pharmacist to identify
a medication regimen that will best meet the patient's needs. We utilize dedicated
software to track prescriptions and refills, which are coordinated through the clinic's
Electronic Medical Record system.
The pharmacists place the education of the patients as their top priority. Whether the
patient is new or returning, they receive counseling from the pharmacist. Through
counseling, the patients understand what medicines they are taking, why they are
taking them, and what to expect. Our pharmacy staff truly develops a relationship
with the patients and can, therefore, better serve their needs.
