Take steps to better health and
earn $100
This is your chance to take steps toward improving your health and well-being and
get rewarded! Participate in the Asuris Small Group Incentive Wellness Program. If
you complete the activities below by <End Month Day>, you’ll earn a $100 gift
card:
1. First, set-up your account or sign in on asuris.com and complete the
online General Health Assessment.
From your member dashboard on asuris.com click on Wellness Tools and complete the
GHA found under Take Action Now.
2. Complete a biometric screening.
Have your blood sugar, cholesterol and body mass index checked. There are several
ways to complete your biometric screening:

a. Get screened at an in-network* doctor’s office and have them fax in the results
using the Physician Results Form. You must register in advance of visiting your
doctor to download and print the Physician Results Form to take to your
appointment.
b. Visit a Patient Service Center. You must register at
https://My.QuestForHealth.com prior to your visit.
c. Complete a home test kit. Home test kits must be ordered by the end of the
second month after plan enrollment. Tests must be completed by the middle of
the following month to meet the three-month deadline.
3. Get your gift card code on asuris.com
Be sure to sign in to obtain your gift card code in your Rewards Tracker. Your gift card
code will be available in your Rewards Tracker for 30 days after your three-month
wellness program campaign ends.

Log in to asuris.com, and click Wellness Tools to begin your wellness journey and
earn $100.
The $100 reward can be received only one time per plan year and may be subject to tax withholding
