Occupational Rehabilitation Occupational Rehabilitation Who to Refer? The CARF accredited Occupational Rehabilitation Program is individualized, focused on return-to-work, and designed to minimize risk and optimize the work capability of the persons served. These services are integrative in nature, with the capability of addressing the work, health, and rehabilitation needs of those served. Such a program provides for service coordination and proactive management of clients with injuries or illnesses. ■ Clients who are currently not working or having difficulty with their job demands or hours ■ Clients whose functional abilities do not match their job functional or cognitive job demands (generally those with medium to heavy physical demand level jobs and/or jobs that require considerable cognitive abilities) ■ Clients who have had difficulty progressing from part time modified work duties back to their pre-injury job ■ Clients who require improvement in their functional tolerances to be able to meet their essential job demands The program identifies, addresses, and reduces, when possible, risks of injury, re-injury, disease, and illness for a safe return to work. The program encourages clients to assume responsibility for the self-management of their own health care plans, in collaboration with all relevant stakeholders. Information about the scope of the services and the outcomes achieved is shared by the program with stakeholders. Program Features ■ Offered in clinic or workplace, 3-5 times per week depending on work schedule ■ One-to-one and group sessions focused on physical conditioning, work simulation and work conditioning to strengthen endurance and tolerances ■ Job specific education to prevent reoccurrence and increase return to work sustainability ■ Early return to work planning and regular progress updates Program Goals ✔ Perform a baseline functional assessment to evaluate client’s current functional capacity and any restrictions, understand the essential physical and cognitive demands of the job and plan treatment to address any gaps ✔ Address functional and cognitive deficits by simulating job duties, providing physical conditioning and work conditioning in a safe environment ✔ Increase client’s knowledge and coping skills regarding their readiness for return to work ✔ Complete regular re-assessments for client’s function to track progress and enable return to work planning ✔ Preparation and implementation of individualized As part of the University Health Network, Altum Health has years of success managing complex and challenging cases and achieving higher than industry return-to-work and fit to return-to-work rates.
