Helen Newberry Joy Hospital - Long Term Care has a memorial fund. The funds are used for elder outings, Christmas gifts, and other Elder needs. Donations to this fund are tax deductible. For specifics, contact Social Work department. Personal Items from home may be brought into the Elder’s room to provide for a personalized environment. Eyeglasses, dentures and hearing aids should also be marked with the Elder's name, which Golden Leaves Living Center staff members can assist. Washer and Dryer are available in Activities Elder Council: Monthly meetings of Elders to plan programs, activities, and share common concerns that assist in improving their quality of life. Elder Planning Meetings: Through the bi-weekly planning meetings, coordinated by the Activity Department, Elders plan holiday events and menus, special events and suggest new programs. Helen Newberry Joy Hospital and Healthcare Center - Long Term Care Elder Petty Cash and Trust Funds: Elders are encouraged to keep no more than $5.00 in their room. Up to $50 can be maintained in the Social Work office for Elders who wish to have spending money (petty cash) available for their use. Amounts over $50 can be put into an interest bearing, group account (trust fund). Care Plan Conferences: -Conducted every ninety (90) days by the interdisciplinary team. The team includes a Registered Nurse, an Activity and Rehabilitation Representative, Occupational Therapist, Physical Therapist, Dietitian, Social Worker and Pharmacist. -Elders and their families are encouraged to attend. Invitations are mailed to the Elder's representative prior to the meeting. -Family members can also participate via telephone. -A plan of care is developed at the care plan conference with goals identified that will assist in providing the Elder the ability to maintain their highest quality of life. Physician Services: Each resident has the right to request their physician of choice from the Golden Leaves Living Center physician team. The physician initially visits the day of admission; then within 30 days; and then every 60 days. Between visits, the physician is available to the nursing staff and elder for any needs that may arise. Pharmacy Service: Full pharmacy services are provided “off-site” with participation in a variety of health plans. Elders who wish to remain with their current medication provider may make arrangements to have their medications delivered to the Golden Leaves Living Center. -Prescription costs and co-pays are the responsibility of the Elder or their financial representative. Therapy Services: Physical, occupational and speech therapy are available when ordered by the physician as part of their treatment plan. Registered Dietitian: A Registered Dietitian supervises meal planning, to recommend special diets and consult on dietary concerns or needs. Diabetic Educator: A Diabetic Educator is on staff providing dietary and medication consultation. Specialty Physicians & Other Off-site Medical Providers: Appointments coordinated by the LTC Nursing and Ward Clerk. Families are encouraged to take Elders to the appointments LTC staff can provide the supervision and transportation. Elders are responsible for the cost of the medical care provided, either through private pay or their insurance companies. No additional fees for LTC staff assistance & transportation. Hospice Services: Hospice services are available within the facility to enhance the care provided to those Elders who are at end of life. Services are available through Northwood's Home Nursing. Licensing & Regulations: The State of Michigan Department of Community Health and the Joint Commission on Accreditation conduct inspections and survey periodically to evaluate care provided to the Elders by the standards set by these agencies. The Michigan Department of Public Health issues our license to operate as a nursing facility. Copies of the most recent surveys from these agencies are posted on the wall outside of the nurses' station. In compliance with State of Michigan regulations, fire drills are conducted once per shift per quarter. An inspection is conducted by the State Fire Marshal annually to determine that the facility is in compliance with all safety rules and regulations.
