Have You…
 Cleaned the ears?
 Trimmed the nails on all four paws?
Included in the Go Home Bag
 Summary form printed off no sooner than the morning the cat goes home.
 Evidence of a neg/neg (or -/-) combo test.
 Rabies certificate and tag if issued. (Rarely, a kitten may not be old enough to get the
vaccine.)
 Microchip tag/info/stickers.
 Vet Check form or SOAP (vets hand written notes) that include everything on the Vet
Check form
 Remove invoices from paperwork unless needed for testing or other medical
documentation.
 Volunteer brochure
 Donation envelope
 Carrier assembly instructions and cardboard tag, if they bought a carrier.
 Sample of food labeled with brand and flavor to help the cat transition.
 Favorite toys
 Snuggly
 Optional articles of interest that may benefit a specific adopter or specific cat such as
“How to Trim Nails or Feline Nutrition.”
 Optional adopter’s handouts found at the bottom of the summary form. (This is nice for
when more than one family member is involved with picking up cat so one can read as
the other drives. ALSO excellent for those adopters that do not navigate computers
easily.)
 Optional calendar (Free ONLY after Feb 15 of each year is any are available.)
Prepared the Carrier
 Attached the carrier label.
 Confirm front carrier clasps are completely secured.
 Cable ties to secure clasps. (Mandatory due to high failure rate)
 Litterbox for all kittens regardless of trip length and for adult cats when trip is more than
4 hours or cat has not used the litter box that morning. (taped to the carrier for stability)
 Favorite toy
 Optional rubber shelf liner to keep towel or padding from sliding.
 Optional incontinence pad for extra padding and absorption in case of soiling.

Added in the Bag for the Trip
 Water and food if trip is more than a few hours particularly for kittens.
 Optional wet wipe, paper towels and plastic bag for clean up. (While ME should carry
this, the adopter on the last leg may not.)
 Small bag of replacement litter should a litter box disaster occur (optional)
 Incontinence pad as a substitute carrier pad in the event of soiling.
