Initially form looks like this.

Here person wants to fill the form, first choose his range, second choose his APC, enter his name and
wants click on calendar icon near date to fill date. In image you can find calendar popup showing.

After choosing date, he wants to choose time also. In image you can find time popup to choose time
Step 4
And lastly he wants to choose kml file from local hard disc.Below image showing popup to choose
file.
Step 5
Image showing all fields with values entered. So it’s time to save or submit the form.
Step 6
Here you clicked save button. So the data is saved. It’s showing popup status.
