LabCorp Clinic Health Screening Option
Directions for completing health screening at LabCorp
The LabCorp health screening is free for owner-associates and you do not need to be enrolled
in a First Data medical plan to participate. The screening will earn you the 2016 Health
Screening Incentive (a $150 deposit into your Health Savings Account in January 2016 or
prorated as taxable income if you don’t have an HSA).
1. The LabCorp option is only available for owner-associates who work in a location without
an onsite health screening. It is not available for spouses/domestic partners.
2. LabCorp screening MUST be completed between May 15, 2015 and September 18, 2015.
3. Schedule your LabCorp appointment by using their online tool or by calling LabCorp toll
free at 888.522.2677 and requesting a clinic that offers “Employee Wellness with Body
Measurements.” To schedule online, visit www.labcorp.com:
a. Select “Find a Lab”
b. Enter your home/work address, city, state and zip code
c. Select “Employee Wellness with Body Measurements” and click Search
d. Select your preferred LabCorp facility and click “Schedule an Appointment”
e. Complete the Patient Information section and select “Employee Wellness with Body
Measurements” as the reason for testing. You will need to include an email address
for patient confirmation.
4. Print the attached LabCorp Form and take it with you to your screening appointment. The
form is your proof that First Data pays for your visit. Without the LabCorp form, you may
be charged for the lab work.
5. Please note
i. Fasting at least 8 hours prior to the screening will provide more accurate
results.
ii. The LabCorp health screening is free, however the online scheduling system
requires that you enter your insurance company name. Your insurance policy
will not be charged as long as you bring the attached LabCorp form.
6. You can see your screening results on www.simplywell.com, under the “My Health” tab 5-7
business days after your LabCorp appointment. If you don’t see your results or you have
questions, please call SimplyWell at 877.991.9355.
7. As a reminder, complete your LabCorp visit no later than 5 p.m., CST on Friday,
September 18, 2015.
Owner-Associate:
To earn the 2016 Health Screening Incentive, you are responsible for visiting your SimplyWell
site to confirm your LabCorp screening results have been entered by the September 30, 2015
deadline. Remember, First Data does not receive your biometric screening results. LabCorp
sends your results directly to SimplyWell. For more information, please see
