SELECTION PROCESS FOR NOMINATIONS:
1. Deadline for submission of this year’s nominations: received by October 15, 2012.
Nominations may be received by mail, delivery service, or email.
2. Deadline for the Hall of Fame Committee to recommend each year’s Inductees to the
Board of Directors will be December 1, 2012.
3. Deadline for the Committee to notify inductees will be December 31, 2012.
4. Hall of Fame Inductees will be presented and honored at the 2013 US Youth Soccer Awards
Gala in Indianapolis.
GENERAL CRITERIA:
1. Nominations for persons to be inducted into the US Youth Soccer Hall of Fame may be
submitted by any person who has been or is a player, coach, referee, or administrator for
US Youth Soccer, an Organization Member of US Youth Soccer, or a member of an
Organization Member. All resumes shall be typed on the Resume form attached to this US
Youth Soccer Hall of Fame Nomination Form.
2. The recognition must be for significant personal achievement in, or contribution to, US
Youth Soccer at the National Level. In addition, the demonstrated, positive effect of such
achievement or contribution must have set a clear and recognized example for the soccer

community to emulate and regard as a model to be followed for personal achievement.
3. Players - must have reached the minimum age of 22 and have distinguished themselves
with an outstanding standard of play and/or performance as a member of US Youth Soccer.
The player must have participated in youth games as a registered member of US Youth
Soccer for at least 6 consecutive years.
4. Administrators, Referees, Coaches, etc. - must have demonstrated outstanding
accomplishments and/or leadership that have made significant contributions to the
improvement of the sport and its participants nationwide. They must have contributed
directly for a period of 6 or more years at the National level of US Youth Soccer.
5. Meritorious Recognition - Nominations for this Recognition are for individuals or
organizations involved in the support of US Youth Soccer in non-playing, nonadministrative, non-coaching, and non-refereeing capacities.
Completed nomination forms shall be sent to the US Youth Soccer National Office, and must
be received no later than October 15, 2012.
