Enter Chart Correction Issue
A Chart Correction issue should be entered and reviewed by the Chart Correction
team if one of the following occurs:
1. A suspected duplicate chart is found for a patient.
2. Charting was completed on the incorrect patient.
3. Staff is notified of a patient death.
Follow these steps to enter a Chart Correction issue:
1. Select to Epic > References > Chart Correction.
Note: If you do not have the References option on the Epic menu, Chart
Correction can be found under Information.
2. If you are reporting incorrect patient information or a duplicate MRN:
a. Select Patient Information in section 1.
b. In section 2:
Last Update: April 4, 2017
i. Enter a contact phone number.
ii. Enter the patient information.
iii. Enter as much detail as possible to assist the Chart
Correction team in identifying and resolving the issue.
3. If you are reporting to HIM that staff was notified of a patient death:
a. Select Deceased Patient in section 1.
b. In section 2:
i. Enter a contact phone number.
ii. Enter the patient information.
iii. Press the F2 key to fill out the wildcards and SmartLists
in the text.
iv. The state requires at least the Date of Death. Including
time and source of information will assist HIM with
keeping all data and records accurate.
