HIGHLIGHTED PRESENTATIONS
Highlighted Presentation: NSTec Daylight Saving Time Predictive Analytics Project
Mr. David Pechulis is a Certified Occupational Ergonomist for NSTec and reports to the Safety and Industrial
Hygiene Manager. In addition to ergonomics, Pechulis has over 16 years’ experience as a business
development consultant providing consulting and training needs to fortune 100 companies including the
financial industry and has conducted seminars on advanced sales psychology. Pechulis has held public office as
mayor and was a U.S. House of Representatives candidate in 2004. Pechulis’ education includes: Colorado
State University, Rochester Technical and Community College, and open courseware through Massachusetts
Institute of Technology. Pechulis presented the following:
Daylight Savings Time (DST) is an event that cannot be changed, there are 3.6 more injuries on Mondays
following DST, and 2,649 more days of work (68%) were lost as a result of those injuries. This is unique to
“spring forward.” NSTec uses predictive analytics – data mining (using industry and research data readily
available) to analyze data for events related to DST, key elements of this analysis are:
 Sleep deprivation
 Worker age related health data
 Complacency
 Historical company data
 Industry data
 Supplemental data
 Environmental factors (weather)
 Projected incidents for DST 2016: 3
Communication is key to reduce DST incidents. Complacency and routine behavior were reviewed. Through
the detailed analysis of NSTec incident history for the week following DST (root cause), NSTec worker
demographics and related aging process, memory retention, specific informative articles to target related casual
factors, and a call-to action, NSTec was able to achieve a company first for 2016 with zero incidents the week
after DST.

1. Lawrence Livermore National Laboratory (LLNL)

LLNL shared the following Lessons Learned: Sunglasses were taken into a laser laboratory and were
used instead of the laser protective eyewear (LPE). The sunglasses and LPE were very similar and it
was not discovered until the individual saw a green spot of light on an optic for approximately two
seconds. LPE must never be stored or placed near other types of eye-wear (e.g., sunglasses, reading
glasses, etc.). LPE must only be in one of three locations: (1) in a designated storage location containing
only the correct LPE for the laser being worked with; (2) on a lanyard worn by the person using the
LPE; or (3) properly donned over the user's eyes. (Lessons Learned and ORPS report NA--LSO-LLNLLLNL-2016-0017)

LLNL also noted that recently Laser Safety Personal Protective Equipment (PPE) worked differently
than expected, individuals saw a small dot when wearing eyewear, instead of not seeing anything. This
was correct for this particular PPE, but was not the expected result. This particular PPE was removed
from the site. (Lessons Learned)
2. NSTec
NSTec shared three Lessons Learned that have been submitted to the DOE Lessons Learned Database and
RL’s OPEXShare:

Cement Truck Falls into Hole Covered with Plywood: A cement truck driver did not stop when the
signal to stop was given by the dedicated signaler, the vehicle continued rolling resulting in the truck’s
passenger front tire resting on a plywood pothole cover which broke through. The cement truck came to
rest on the front axle and front frame. Lessons Learned: If using covers for trip and fall hazards around
vehicular traffic, then mark the area as walking surface only and have stanchions and flagging on them
to avoid vehicular traffic. This event shows that a lack of situational awareness could result in vehicle
and/or property damage, and possibly personal injury. (Lessons Learned)

Verification and Validation Process for Checklists Used for Unique Experiments: The Best Practice of
mandating three walkthroughs of experimental checklists prior to the beginning of operations will
ensure safe operations; especially critical experiments involving the use of highly hazardous materials.

3. Thomas Jefferson National Accelerator Facility

TJNAF discussed a lesson learned during annual inspections of unique power supplies. Some units were
involved in a flood which were kept, red tagged, and put in storage. The concern with these units is that
they could be used for parts when they should not be. The inventory needs to be carefully controlled as
these red tagged units’ parts should not to be placed into critical systems.

DISCUSSION

T.J. Lyons, Gilbane, presented a lesson learned regarding sports equipment and small children or
animals. Lyons was taking pictures of his family at a park on a soccer field when one of the younger
children turned around, walking into the net and got her head caught in a net (photo below). Lesson
learned is to have the appropriate organization take the nets down when not in use.

Security Updates to DOE Databases: User accounts for DOE databases (including Lessons
Learned, ORPS, CAIRS and more) will be disabled after 60 days of inactivity, after which users
will be required to call or email the AU help desk to re-activate
o Users will be required to change passwords every 60 days. If the user fails to change the
password within the given 60 days, they will be required to call or email the AU help
desk to get a temporary password assigned
o Accounts will be deleted after 1 year of inactivity

o AU-23 started the development of a strawman of the Handbook, a redline strikeout
version will be sent to the LL working group soon

Update of DOE O 210.2A
o With the new process in place for reviewing directives, all directives are on hold, except
the directives chosen to go through the pilot process- more below on this initiative.

Update of DOE O 232.2- DRB Pilot Process
o As part of the Laboratory Operations Board’s initiative to reform the DOE directives
program, two directives have been identified as candidates for piloting improvements.
These include DOE Order 483.1A, Cooperation Research and Development Agreements
and DOE Order 232.2, Occurrence Reporting and Processing of Operational Information
o Ruocco and Mike Schlender from PNNL are co-chairing DOE O 232.2’s pilot process
o An Integrated Project Team (IPT) has been formed to work on this process, and the kickoff meeting was held August 4
o Once a decision has been made on the path forward, a Decision Memo will be sent to the
IPT and Operations Committee
o Co-chairs will coordinate with MA throughout the pilot process as MA is reviewing and
updating DOE Order 251.1C, Departmental Directives Program
o Ruocco will provide updates to the OEC on this pilot every webinar
o Expected completion and publication of DOE O 232.2 is December 2016
o ACTION: Ruocco will send out the IPT Member List to the OEC for their awareness

