To provide a purposeful relationship with individuals and their family/whanau to enable them to activate their goals and aspirations; increase their quality of life and move towards greater interdependence. Task Uphold the Organisations vision and Philosophy towards Te Tiriti o Waitangi. Liaise with Kaiarahi and Iwi as appropriate. Outcome Culturally appropriate service delivery to clients. Improved bi-cultural practice. Communicate with other professionals/agencies as appropriate Liaise regularly and attend meetings. Maintain weekly client updates through written source letter/email. Paramountcy Work to child safety and reporting Policies Ensure policies and procedures reflect the Paramountcy of the child or young person is the main priority Regular training in Non-violence crisis intervention De-escalation training Have attended training in recognising and reporting Child abuse Have knowledge of all Gateway Housing Trust/Snapshot policies and procedures relating to the safety and wellbeing of child and youth clients That information is shared appropriately. Networks and relationships between GHT/Snapshot Services and other agencies are developed and maintained. All agencies and Snapshot Services have the necessary information for best outcome for their mutual clients and their families/whanau. Follow Gateway Housing Trust philosophy, ethics and boundaries. All documentation related to individuals is accurate and current – both electronic and hard copies. Administrative systems are used in accordance with Gateway Housing Trust Policies and procedures. Clients receive entitlement and are aware of their rights. Outcome The employee works within Gateway Housing Trust policies and procedures. The employee works safely and competently. Administrative tasks are properly completed and in a timely fashion. All Events/Incidents are recorded on GHT Quality and Risk Database. Maintenance of Trust Assets Be responsible for specific resources e.g. vehicles. Advise Team Leader of deterioration and/or damage to assets. Under guidance, arrange maintenance and repairs. Adhere to Gateway Housing Trust Health and Safety policies and procedures. Report any identified hazards Follow Gateway Housing Trust Infection Control policy and practices. Participate in Snapshot team development – planning and team building. Share Case Loads as required. Demonstrate empathy towards individuals who experience social disadvantage. Provide ongoing professional support to clients. Use crisis intervention skills and appropriate follow-up Provide advocacy. Develop knowledge of local and regional agencies and services that are of benefit to Service Users. Lead, motivate and encourage. Assist individuals to access community resources e.g. – education services, sporting groups, budget advice, transport etc. Liaise with government organisations – CYFS, Police, Department of Work and Income, Education, Courts etc. Assist with life skills and advocate when appropriate with government agencies. Individuals achieve tasks that lead them to their goals and improve their quality of life Individuals are referred to agencies and services as appropriate. Understand family/ whanau issues. Understand how systems can influence and impact on the wellbeing of young people. Discern the difference between the needs of the individual and that of the family/whanau. Creating opportunities to include family/whanau. Outcome Individuals are given opportunities and support to gain an understanding and empathy for victims. Clients have an increased understanding of the importance of community spirit. Client are focused on future goals, education and work. Family/whanau are included in decisions and are involved in supporting their family member in achieving their goals whenever possible. Increased Family/whanau involvement. Family have a greater understanding of their young person’s issues and support needs.
