Principles for the future
1. Care delivered through a network of hospitals and community
services
2. More seamless care close to or in the patient’s home where
safe and effective, access to urgent and community care 24/7
3. Patients only admitted to hospital where it is no longer safe or
effective for them to be cared for in the community
4. Access to specialist opinion 24/7 where this improves outcome,
e.g. heart attack, stroke, trauma, or internal bleeding
5. Planned care organised so there is no unnecessary waiting, no
cancellations and patients not exposed to risk of infections

LOCAL HEALTH CHIEFS SLATED FOR NOT ACTING
• More of our older residents are finding themselves in hospital needlessly when they
could be cared for in their own homes of the community
• Bed blocking is worse than ever with delays on discharge and delays for those needing
urgent admission
• Elective surgery cancellations and delays are high as hospitals try but fail to balance
the needs or emergency and planned care
• There is a significant lack of consultant cover at all of our A&Es meaning treatment by
senior staff is the luck of the draw for patients
• NHS Agency and Locum staff rates are higher than ever before across the country
• Reports show quality levels are lower than a decade ago as standards are slipping
across our hospitals

1.Urgent/on the day needs of complex/vulnerable- met within
care hubs or at rapid assessment bases where appropriate
2. Urgent/on the day needs of the rest- either within hub if
capacity as usual or at central point at DMH site, but a primary
care service-appt or walk in
3. Rapid assessment areas supporting the primary care central
response team with rapid specialist opinion and investigation
