Engagement events May 2016 Thank you for your interest • Today is part of a series of events to get input of local people into developing services • The format is round table discussions, led by a facilitator and recorded by a note taker. • This is NOT part of a formal consultation. No options are under discussion. • We plan a formal consultation on options from November 2016 For more information about the Better Health Programme: www.nhsbetterhealth.org.uk • Twitter: www.twitter.com/NHSBetterHealth • Facebook: /nhsbetterhealthprogramme Agenda • • • • • Sharing first draft of local plans for Darlington Background to Better Health Programme Video – principles and draft framework Workshop/discussion Next steps Increasing elderly population Specialist teams provide better outcomes Increasing long term conditions Better Health Recognised shortage of specialist skills Supporting independent living Greater financial stability Durham, Darlington and Tees Vision: “Meeting patient needs now and future proofing for the coming generation with consistently better health and social care delivered in the best place and within available resources.” Durham, Darlington and Tees Our vision is in line with the national vision • Highly responsive, effective and personalised services outside of hospital for people with urgent but non-life threatening needs. • People with more serious or life threatening emergency needs treated in centres with the very best expertise and facilities in order to reduce risk and maximise their chances of survival and a good recovery. • Planned care in an environment separate from emergency care which avoids unnecessary delays and cancellations. Care delivered through a network of hospitals and community services 2. More seamless care close to or in the patient’s home where safe and effective, access to urgent and community care 24/7 3. Patients only admitted to hospital where it is no longer safe or effective for them to be cared for in the community 4. Access to specialist opinion 24/7 where this improves outcome, e.g. heart attack, stroke, trauma, or internal bleeding 5. Planned care organised so there is no unnecessary waiting, no cancellations and patients not exposed to risk of infections Durham, Darlington and Tees Draft framework of care Better Health Programme Jan 2016 Durham, Darlington and Tees Your views • Do you support the principles of care as a reasonable direction of travel? • Do you support the draft framework of care as a reasonable direction of travel? • Any ideas or questions you have to enhance the draft framework for care, or issues raised? • What are your priorities for improving care for patients and for decision making criteria? • What are your thoughts about the blueprint for primary care and healthy town proposal? • Any other comments/suggestions? Additional Slides (used at the Darlington events) LOCAL HEALTH CHIEFS SLATED FOR NOT ACTING • More of our older residents are finding themselves in hospital needlessly when they could be cared for in their own homes of the community • Bed blocking is worse than ever with delays on discharge and delays for those needing urgent admission • Elective surgery cancellations and delays are high as hospitals try but fail to balance the needs or emergency and planned care • There is a significant lack of consultant cover at all of our A&Es meaning treatment by senior staff is the luck of the draw for patients • NHS Agency and Locum staff rates are higher than ever before across the country • Reports show quality levels are lower than a decade ago as standards are slipping across our hospitals Primary Care Blueprint Primary Care Blueprint Primary Care Blueprint Summary of urgent response 1.Urgent/on the day needs of complex/vulnerable- met within care hubs or at rapid assessment bases where appropriate 2. Urgent/on the day needs of the rest- either within hub if capacity as usual or at central point at DMH site, but a primary care service-appt or walk in 3. Rapid assessment areas supporting the primary care central response team with rapid specialist opinion and investigation What does this mean for Darlington? • • • • • Practices working closer together to share expertise Improved access for primary care advice 7 days a week Better communication between the teams looking after you Services wrapped around patients based on need You won’t always need to see a GP – other health and wellbeing professionals will be available – Physiotherapists – Nurse practitioners – Pharmacists – Mental Health support workers What doesn’t it mean? • That practices are closing • That continuity of care for Long Term Conditions will be lost • That you will lose your named GP Primary Care Blueprint Possible Darlington Healthy New Town What will this mean to people in the East Darlington Village?
