Upon the completion of an agility trial, the Superintendent/Event Secretary shall complete a copy of this form for each event (one
event per form) and send it with the marked and signed trial catalog along with the necessary fees so as to reach the AKC® office
within seven (7) days after the close of the event. Each question should be answered completely.

Event secretaries are allowed to secretary up to twelve (12) events without a secretarial fee. After acting as an event

Effective January 1, 2013: Any person or entity who serves as secretary for thirteen (13) or more AKC Agility Trials in a
calendar year is required to be approved as an AKC Licensed Agility Trial Secretary.
