Packages and Fees
ADHD Consultation Session ($150 per hour)
 Individual session with caregivers only
 This option is available for families with children who have previously been diagnosed
with ADD/ADHD
 Includes education regarding diagnosis, biological factors, treatment options, academic
recommendations, and community resources
 Helpful handouts, referrals, and book recommendations will be provided
ADHD Testing Package ($830)*
 Clinical interview of caregivers and interview/observations of child (1 hour)
 ADHD testing (1 hour) – includes general behavioral and emotional assessment forms
for caregivers and child, ADHD assessment forms for caregivers and child, and
computerized assessment (TOVA) with child only
 Scoring, interpretation, and report writing (2 hours)
 Testing feedback session (1 hour in person or by phone) – includes comprehensive
written report with diagnosis and detailed recommendations, additional helpful
handouts, referrals, and ADHD education for caregivers
IQ Testing Package ($450)*
 IQ testing (2 hours) – intellectual/cognitive assessment of child using the Wechsler
Intelligence Scale for Children, Fifth Edition (WISC-V)
 Scoring, interpretation, and report writing (1 hour)
 Testing feedback session (30 minutes in person or by phone) to discuss results and
provide recommendations is optional with additional cost of $100
Educational Testing Package ($1,280)*
 Clinical interview of caregivers and interview/observations of child (1 hour)
 IQ testing (2 hours) – intellectual/cognitive assessment of child using the Wechsler
Intelligence Scale for Children, Fifth Edition (WISC-V)
 Academic testing (1 hour) – academic achievement assessment of child to evaluate
mathematics, reading, and writing skills using the Woodcock-Johnson Tests of
Achievement, Fourth Edition (WJ-IV)
 Scoring, interpretation, and report writing (3 hours)
 Testing feedback session (1 hour in person or by phone) – includes comprehensive
written report with diagnosis and detailed recommendations, additional helpful
handouts, referrals, and guidance for caregivers

These services are provided by Lindsay Asawa, Ph.D. in Sugar Land, Texas. Dr. Asawa completed her
doctoral degree in clinical psychology at the University of Nebraska-Lincoln. She completed her clinical
internship at Baylor College of Medicine and her post-doctoral fellowship at Children’s Medical Center
Dallas. Dr. Asawa specializes in working with children and families to evaluate and address many of the
common problems that arise in the home and school settings. She has extensive experience evaluating
and treating children and adolescents in private practice, university clinics, and hospital settings. Dr.
Asawa has also developed and facilitated workshops and treatment groups for both parents and
children. She utilizes evidence-based assessments and treatment recommendations, incorporating both
cognitive-behavioral and family systems approaches. Dr. Asawa is an out-of-network provider for private
insurance plans. However, families can be provided with the necessary paperwork to file a claim with
their insurance company. Dr. Asawa has been a licensed psychologist in the state of Texas since 2009
(License #34285).
