Community Member Survey Guide
Explaining background:
Our team is participating in a project called TRUE. The objective of TRUE is to optimize
appropriate referrals to hospice to improve utilization of hospice and increase the days of care
per patient (i.e. length of stay). TRUE is bringing together hospices, health care organizations,
and community members in [our] community to discuss ways this could happen here.
In order for our team to identify potential barriers to access and enroll in hospice in our
community, your candid responses are most appreciated. Your responses will be kept in strict
confidence and your name will not be requested for this conversation.
