In 3rd Quarter 2015, TDS TELECOM will decommission its Class 5 office switch in the Wingate, IN
(WNGTINXADS0) rate center.

All customers currently served by WNGTINXADS0 have been migrated to the WNGTINXA0MD end office which
subtends the CYTNINXAGT0 gateway office.

TDS Telecom will be contacting impacted carriers to coordinate disconnect orders for any trunks remaining on
WNGTINXADS0. Once all carrier trunk groups have been disconnected, the switch will be decommissioned.

Carriers requesting new or augmented Direct End Office Trunk groups to the CYTNINXAGT0 gateway office may
order those through the TDS Telecom Access Service Center. The SS7 Point Code for this office is 223-008-012.
The CYTNINXAGT0 gateway office subtends the AT&T IPLSIN0110T tandem for FGB/D.
