Twelve Latest Passport photographs {3 x 3cm (6 Nos.), 2.00 x 2.5cm (6 Nos.)} (Sky-blue Background) duly
attested (Backside). By Government officer (Grade-17 or above).
Certificate of Vaccination of Tetanus and Hepatitis “B” (specimen available on college website).
Student’s N.I.C. or B- form copy.
Father/ Guardian’s N.I.C. Copy.
Surety Bond (Specimen attached) on a stamp paper of worth Rs. 100/- duly attested by the Civil Judge (specimen
available on college website).

found wrong in any way at any stage. Admission is also be cancelled/ changed if any error or omission is detected at any
stage.
d). Apart from this, all the rules and regulations contained in the prospectus of the Medical Colleges of the Punjab and changes
from time to time are applicable and they are binding on you.
f). If you accept, please report to the office of the undersigned alongwith this call letter, fee and documents mentioned above
on or before 04-12-2013. If you fail to pay the fee alongwith required documents and certificates within the stipulated date,
your selection will be cancelled. In case you are not willing to accept these terms and conditions, please submit your reply in
