Purpose
Use this form to capture required data from your incentive program mobile market customers. A completed form will have:

# of new customers (p. 3)
# and $ of SNAP transactions and Incentives distributed (p. 3)
# of producers/vendors that contributed to the mobile market offerings (p. 4)
Answered Post Market Questions (p. 4)
Mobile Markets have two options for reporting data:
 Option 1 – Minimum Required. Collect aggregate data (total sales) for all of your stops combined for day.
 Option 2 – Advanced. Collect sales data by stop (this is welcomed but not required).
When you have completed this form for your mobile market stops, enter the data into the FM Tracks website (instructions below). If you have questions about

Step 1: Complete Mobile Market Route/Stop(s) & Date(s) (p. 2)
Step 2: Record Individual Sales and Transaction Information in Transaction Log (p. 2)
Step 3: Add up all of the transaction totals in Totals: Payments & Incentives (p. 3)
Step 4: Track sales by site in the Sales by Stop Log (p. 3) – Optional
Step 5: Complete Post Market Questions (p. 4)
Step 6: Enter data into the FM Tracks website


Enter data into the FM Tracks website at a minimum for each week at: www.fmtracks.org within the ‘Enter Daily Sales & Data’ section
Confirm accuracy and completeness of sales data within the ‘Run Reports’ section
 Use Transactions Log (p. 2) to enter ‘Transactions’
 Optional: Use Sales by Stop Log (p. 3) to track sales by mobile market stop
 Use Post Market Questions (p. 4) to complete the ‘Post Market Report’

FM Tracks keeps track of new customers using FM Tracks IDs
Creating FM Tracks IDs
Your Mobile Market has 2 choices for formatting IDs. CHOOSE ONE AND USE IT
CONSISTENTLY:
 First and last initials on the customer’s EBT card and last 4 digits of the EBT Card OR
 Customer’s first and middle initials and birth month and day (MMDD)
FM Tracks IDs should always be 6 characters, for example: DB1017
