Sports Achievement made during the Award Period (1/5/2016 - 30/4/2017) would be considered.
Please email the completed electronic nomination form together with one passport size photo and two action
photos to [email protected] (HKI&K Region) / [email protected] (NT Region).
The signed copy of nomination form should reach the Federation office by hand or by post at Rm 203
(HKI&K Region) or Rm 102 (NT Region), 7 Carmel Village Street, Homantin, Hong Kong on or before
17:30, 28th April 2017. (Please mark “Grantham Outstanding Student Athlete Awards” on the envelope)
An acknowledgement will be sent to you by email within 1 working day after receipt of your application. If
you do not receive any acknowledgement email 3 days after submission of application, please contact the
Federation Office at 2711 9182 (HKI&K Region) or 2711 2823 (NT Region).
