Are you and your team ‘Thinking Family’ yet? The Family Focus Programme aims to support families who have multiple and often complex needs through a whole family approach. York has already supported 315 York families to improve their lives and in April 2015 the programme was expanded and is expected to support a further 1,030 families over 5 years. The expanded programme will support families with a wider range of needs, including: 1.Parents and children involved in crime or anti-social behaviour. 2.Children who have not been attending school regularly. 3.Children who need help: children of all ages, who need help, are identified as in need or are subject to a Child Protection Plan. 4.Adults out of work or at risk of financial exclusion or young people at risk of worklessness. 5.Families affected by domestic violence and abuse. 6.Parents and children with a range of health problems. Families eligible for the programme are identified through the merging of data from partners across the city and it is then the role of the Family Focus Team to ensure that each family identified has a lead practitioner from a relevant service working with them. What does a Lead Practitioner for a family on the programme do? 1. Ensures a family have given consent for their involvement in the programme 2. Undertakes a whole family assessment, with the support of partners, to identify any issues for every family member. 3. Develops with the family and relevant agencies a whole family action plan to address the needs of every family member in a timely and co-ordinated manner. 4. Co-ordinates the services/interventions to avoid the family getting overwhelmed. 5. Through regular review meetings, monitors the progress of the action plan with the family and the team around the family. 6. Provides relevant information to the Family Focus Programme leads, in line with the requirements of the national programme . 7. Develops a robust exit plan when stopping work with a family to ensure ongoing appropriate support is available as required to allow the family to continue to sustain progress and build on this.
