MENTOR CHECKLIST WEEK 11-12 AIMS: Agree Specific Measurable Achievable Realistic and Time related goals Identify barriers/solutions Mentor’s Name Date Young Person ID Contact time (in hours) Did I…. Week 11 Week 12 Identify 3-4 goals that the young person wishes to work towards based on sessions to date? Ask the young person what they think will be difficult about achieving their goals? Consider what precedes the behaviour the young person is trying to change? (e.g. late nights on the Xbox – not getting up in time for school or heavy criticism from a significant adult – conflict in the family home/school) Consider if their goals are realistic? Build consensus around goals if unrealistic Consider distance travelled already by the young person to help increase motivation? Consider how the young person’s motivation will be maintained over time, build in appropriate rewards? Take time to explain to the young person to expect setbacks, this is part of learning? Consider any heroes the young person has i.e. sport – how they achieve goals, takes time, practice, determination, a coach (sometimes)? Consider whether the young person will need additional support to achieve goals i.e. parents/carers on board and/or advocacy brokering from Programme Manager? Assess what the expected timeframe is for achieving them and introduce notion of reviewing these? Again reaffirm this is a two way process and we will go at the young person’s pace? Continue to model prosocial behaviour? Continue to have fun/share interests?
