GUIDANCE NOTES ON COMPLETING YOUR APPLICATION FORM

Please provide ALL the requested information. Failure to complete this application form in full may
result in us returning your form to you.

We cannot process any application forms where a signature is not present, so please ensure you
have SIGNED and DATED your form. If you have submitted your form electronically we will send a
summary of your booking details to you, which must be signed and returned before being considered.

Where provided, your email address will be used to notify you of issues regarding our facilities (such
as closures during adverse weather conditions etc).

Please specify whether your booking is required on a “CONSECUTIVE” or “ALTERNATE” weekly
basis. To assist in this choice, the following rules typically apply:a)
An individual team playing league fixtures on a Saturday will require a booking on an
ALTERNATE SATURDAY basis. It is classed as an “alternate” weekly booking as there
should typically be alternating home and away fixtures.
b)
If a club has more than one team playing on a Saturday and both teams require the same
home pitch, this will require a CONSECUTIVE SATURDAY booking. This is because a home
pitch will be required every week (ie, one team plays away as the other team plays at home).
