Clicker Question
• What is the critical thinking competency
specific to nursing practice?
• a. Diagnostic reasoning
• b. Clinical Decision Making
• c. Nursing Process
• d. Scientific Method
NURSING PROCESS
• Assessment
• Diagnosis
• Planning
• Implementation
• Evaluation
Break into Groups
• Discuss how each piece of the nursing
process helps you to solve your
patients health problem.
Step One: Assessment
• Collect data
• Verify data
• Organize data
• Identify patterns
• Record & report
Step 2: Diagnosis
• Analysis of
assessment data
leads to problem
identification
• NANDA Label
• What are novice
nurses responsible
for?
Ineffective airway clearance related
to……
Step 3: Planning
• Set priorities
• Apply standards
• Identify goals &
outcomes
• Determine
interventions
• Record the plan
Clicker Question
• Which patient would you assess first after
finishing morning shift report?
• a. The patient who is being discharged in the
afternoon and needs medication education.
• b. The patient asking for pain medication
• c. The patient who is going to the OR in 30
minutes
• d. The post-op patient with a morning BP of
90/50
What are the priorities?
Maslow’s Hierarchy of Basic
Human Needs
Step 4: Implementation
• Put your plan into
action
• Perform the
interventions
• Note patient
response to
interventions
• Record & report
Step 5: Evaluation
• Did the plan work?
• Was goal achieved?
• What was the
outcome of the care
provided.
• Stated in
measurable terms.
• It’s all about
outcomes!
Case Scenario
• Mrs. Jones is an 89 yr old male who
was admitted with pneumonia. He has
complained that his throat and mouth
are very dry. The nurse notes that he
has a temperature of 101 and has dry
mucous membranes. He is allowed to
drink, but does not want to bother the
nurse for fresh water. His urine output
is low as well.
Now lets write the plan down!
Concept Map Steps
• Place your main issue/problem in the middle
• Determine key problems/concepts that have
a direct relationship to the main problem
• Add clinical data to appropriate problem
boxes
• Draw lines between related problems. Label
with a nursing diagnosis
• Identify goals/outcomes
• Add interventions
• Evaluate patient response to interventions
•
