Humble ISD Benefits
New Hires with a date of hire of 08/01/2015 or before
08/01/2015 will have TWO benefit enrollments to complete.
1. The first enrollment will be your initial benefits
enrollment period that must be completed within 31
days of your date of hire (first day at work). Your
benefits will begin 08/01/2015 and end on 08/31/2015
due to the end of the plan year.
2. The second enrollment is OPEN ENROLLMENT for
benefits that begin on 09/01/2015 (Please refer to the
New Hire Benefit Enrollment Processing Guideline
Requirements with Humble ISD included in your New
Hire Packet).

What you need to know:
•Benefits for Humble ISD run on a PLAN YEAR: The benefit
plan year begins every year on September 1st and ENDS on
August 31st.
•Your benefit elections will remain in effect for the plan year
unless you experience a Qualifying Event that will allow a midyear change.
•You can expect to receive your insurance cards within 3-4
weeks AFTER completing your benefits enrollment.

Security Basic Facts sheet, “Just over 1 in 4 of today’s
20 year-olds will become disabled before reaching

You can elect up to maximum amount of $375,000 for your spouse, but you will need to
complete an EOI to obtain coverage greater than the $30,000 GI amount.
Rates are based on Age. As you get older, your premium can increase.
Once you reach age 65, your benefit amount will reduce 35% of the initial amount. One
you reach age 70, the benefit amount will reduce 50% of the initial amount.
Accidental Death and Dismemberment (AD&D) with Aenta

Accidental Death and Dismemberment pays an addition benefit amount in the event a
death were to occur from a covered accident.
A specified benefit amount is also covers any eligible bodily loss due to a covered
accident.
Employees may choose to elect an amount equal to the coverage elected for the Term
Life coverage. (Employees must be enrolled in the Term Life to elect the AD&D
coverage)

Loyal American Cancer Plan
• Your cancer insurance is through Loyal American.
• Cancer insurance is designed to help cover expenses that a
medical insurance won’t typically cover such as
transportation, lodging, prosthetics, hair pieces, etc.
• Your cancer insurance is Guaranteed Issued, which means
even if you have had cancer treatment before, you are still
eligible for this benefit.
– Please note there is a 12 month waiting period for any preexisting conditions.

Two Options, Plan A or Plan B.
Both plans offer an Annual Cancer Screening benefit, up to $100 for
Plan A; up to $50 for Plan B.
First Occurrence benefit, Daily Radiation, Chemotherapy,
Immunotherapy, Experimental Treatment benefit available in both
plan options.
Transportation and Lodging benefit available.
Specified Disease Rider, covers 38 Specified Diseases, if an insured
person is first diagnosed with one or more covered Specified
Diseases and is hospitalized for the definitive treatment.
