Education Recognition Ceremony Guidelines
1. Students must be registered Lyackson First Nation Band Members in order to receive any
incentive the Chief and Council give to the students.
2. Students in K – Gr. 8 must submit a report card from the current school year (September
to June). If a student between K – Gr. 8 has misplaced their report card then the
parent/guardian may complete the Education Declaration Form on behalf of their child.
3. Students in Gr. 9 – Gr. 11 must submit their 3rd term or final report card in order to
receive their incentive. If a student in Gr. 9 – Gr. 11 has misplaced their report card then
they can have an Educator (e.g. teacher, school councillor) complete the Education
Declaration Form on the students’ behalf in order for the student to receive their
incentive.
4. Grade 12 Graduates must submit their final report cards in order to be recognized as a
graduating student. If a Grade 12 Graduate has misplaced their report card then they may
submit their Dogwood Certificate or have an Educator (e.g. teacher, school councillor)
complete the Education Declaration Form on the students’ behalf in order for the student
to receive their incentive.
5. Post Secondary students must submit their most recent transcripts in order to receive their
incentive.
6. If Students have not submitted their report cards and/or transcripts by the date of the
Education Recognition Ceremony, they will have until September 30th to do so in order to
receive their incentive.
7. Cheques will be given directly to the students. If the student is not in attendance then the
cheque will be mailed to his/her address on file in the office. If a student knows in
advance they will not be able to attend the ceremony they can notify the office and their
cheque will be mailed during the week prior to the ceremony.
