2012 VOLLYBALL REGISTRATION
INSTRUCTIONS:^ Team Captains are to complete this registration form and return it, with the league fee
of $300.00, to Harpoon Eddie's no later than May 24th. Teams will be accepted on a first come, first serve
basis. We are scheduling a league meeting and sign ups on Thursday, May 10th at 6:30 p.m. We will be
holding a practice night on Thursday, May 17th for those who would like to come down and practice before
the league starts. The Volleyball league will begin on Thursday, May 24th. There will be two nights of
playoffs, unless there needs to be a rain date. On August 30th, there will be a year end party for all players.
CANCELLATIONS: Due to the weather will not be made until 5:00 p.m. at which time the captain will be
notified.

Team Registration: Teams must have a minimum of 4 players and a maximum of twelve. All
correspondence will be sent to the team captain. Teams must have at least two members of the opposite sex
on the court at one time. Players can play on only one team per night. All teams must have 4 players on the
court or game will be forfeited.
PAYMENT: All league fees must be paid in cash or check prior to the start of the league. Checks should be
made payable to HARPOON EDDIE'S. Teams that have not paid in full by opening night will not be
allowed to play. NO EXCEPTIONS!
LIABILITY RELEASE: All league participants must sign the liability release on the attached form.

To participate in the HARPOON EDDIES VOLLEYBALL LEAGUES competitions for good and valuable
considerations, I the undersigned participants, hereby agree to the following terms and conditions to govern my
participation in stated activities.
1. I hereby verify and certify that I am sufficiently competent and skilled in the sport or activity I have
chosen to participate in, as indicated by my choice above, and hereby warrant that I will or will
have, before the competitions take place, inspected the competition areas to assure that I feel
confident that they are safe, fit and proper condition for the competition. Further, I agree that I will
not participate in the competitions unless said areas are reasonable, safe and within my proficiency
level.
2. 1 hereby assume all risks of injury or loss resulting from participation in the competition, which
may exist for any reason of participating in any of these competitions.
3. I hereby, for myself, my heirs, administrators, executors, and assignees agree to release, remise, and
forever discharge HARPOON EDDIE'S and any other persons connected with the competition, and
each of their respective heirs, legal representatives, or assigns, and from any and all claims,
demands, liability rights or caused of action of whatsoever kind of nature which I may have, arising
from, or in any way connected with any injuries, losses, damages, disability, sufferings, property
damage or loss, or the results thereof, which may be sustained by be or in connection with or
arising out of my participation in this event. I understand that this paragraph constitutes a covenant
on my to part fully discharge the above named parties of and from any liabilities for injuries or loss
resulting from my participation in the "HARPOON EDDIE'S LEAGUES"
