Round 3 NHS Leeds North CCG Third Sector Health Grants NHS Leeds North Clinical Commissioning Group (CCG) is an ambitious organisation that strives to improve the health and wellbeing of local people. Over the past 18 months the CCG has committed over £900,000 of non-recurrent funding to deliver an exciting new programme to help deliver its ambitious plans to improve health and wellbeing, prevent ill health, improve the health of the poorest the fastest and ultimately enable local people to lead healthier lives. This is underpinned by the recognition that the third sector is a significant asset to the city. We are now launching the final round with the remaining monies. Aim of the grants programme The programme seeks to support Leeds North population–based projects which must be able to achieve demonstrable, measurable health outcomes and impact. The first two rounds have funded a range of projects that complimented our Social Prescribing Service “Connect Well”. It is the CCG’s intention for this 3rd round to further build on projects that will support our Social Prescribing Service or meet specific Leeds North CCG priorities. Additionally, the CCG has changed the size of grants and the length of time for which they are awarded. Our intention is to invest money in third sector (voluntary, community and faith) organisations in order to build evidence of health impact which, if sufficiently robust, has the potential to enable resource shift within the local health system. In this third round, the programme is looking for applications which demonstrate the following: Strong health outcomes and impacts in the highest priority areas for Leeds North CCG (see Key priorities overleaf) Building or strengthening capacity to support the Leeds North Social Prescribing Service “Connect Well” Developing services that will reduce demand in other areas of NHS funded provision Evaluation using recognised health tools and measures Activity that has not already been evaluated in the Leeds North context A more resilient way of providing services without additional monies. Grants of £5,000 to £25,000 are available. In partnership with Funding will provide new opportunities to the third sector (voluntary, community and faith) organisations to: Develop innovative ways of working in the Leeds North area to meet the identified health and wellbeing needs of the population registered in this area Gain understanding of the health and wellbeing needs of the communities in the Leeds North area—this could involve gaining views and opinions from patients/service users, families, carers, schools, health workers and health providers Deliver new activity to fill identified gaps in current service or community provision, increase the reach of existing activity or improve links to existing provision Help people - in particular those in vulnerable groups — to access existing local services, support and community provision Support partnerships between organisations to tackle problems or improve effectiveness Focus on how third sector organisations can provide services, or how existing health services can be provided differently, to reduce the demand in other areas and demonstrate this Focus narrowly enough on a specific activity to evidence measurable impact and evaluate Work in partnership with NHS providers including GPs Key priorities Reduce deaths from Prevent Improve the care Improve support for cancer, heart and alcohol related people receive at the people with long lung disease diseases end of their life term conditions Encourage people to manage their own health better Improve access to healthcare services Local responses to Leeds Health and Wellbeing Board priorities Developing capacity that further complements the Leeds North CCG Social Prescribing project including befriending, support with accessing new activities and specific activities aimed at people with long term conditions who are most socially isolated. Find out more www.leedscf.org.uk Eligibility criteria The primary activity of your group needs to benefit people living in the area covered by Leeds North CCG / or registered with a Leeds North CCG Medical Practice. The fund is about communities and community demand not about organisations leading delivery. You will need to demonstrate strong evidence of involvement of patients / direct service providers from Leeds North area and the potential for sustainability beyond the end of the grant (through commissioning, personal health budgets or other income). The programme is open to third sector organisations i.e. : An unincorporated voluntary or community group with a constitution A registered charity or registered charitable incorporated organisation A registered society, community interest company OR a non-profit distributing organisation (such as a social enterprise that reinvests surpluses for social benefit), provided they are able to receive grants. We will also accept applications from: Third sector organisations working in partnership with GPs, practices, academic institutions or consultants around evidence and data gathering or research provided, that the lead applicant is a third sector group and the majority of costs relate to activity carried out by the third sector group Third sector groups working with statutory organisations where the third sector is the lead party and the recipient of the grant Leeds based consortia or infrastructure organisations, as long as the activity for which you are seeking funding has a clear demonstrable impact on improving service delivery for frontline organisations. Exclusions The programme will not provide funding to: Cover current running costs or replace funding streams that are ending Duplicate previous learning or provision that is already seen as of proven or disproved value Projects that have been previously funded by the CCG unless for a different proposal Capital request to buy capital assets (e.g. vehicles) refurbish assets not owned by the group (e.g. rented space) unless there is a very strong link to deliver direct health outcomes Projects where the health outcome is by product not the primary focus of the activity (e.g. benefits advice; sports clubs unless specific additional health benefits are identified) Statutory organisations, such as a school or local authority, as the main applicant Individuals or sole traders Private businesses or organisations that make a surplus or profit which is not principally reinvested for social benefit Groups applying on behalf of another organisation. Grants can take on a number of forms: including - but not limited to -: Running a project or pilot that results in a new service delivery offer Developing services that can be funded through individual personal health budgets Delivering services in a more cost effective way, or closer to home Working in partnership for the first time, or working with different partners Peer-led participatory activity involving local people in bringing about change in a specific geographic area Pilot projects using local people to deliver activity in local communities Local people / social entrepreneurs with ideas that would improve the health and wellbeing of local people where they live Activity that meets health needs of a specific demographic group or geographical area Moving into an area of work where there are current gaps in provision - for example, developing a community service identified as a gap by the Social Prescribing Service. Grants can be used for: Project costs, including capital equipment where directly linked to delivering direct health outcomes Developing/commissioning external evaluation in the language and currency required by Leeds North CCG Staff costs (where clearly shown as being extra to on-going duties) Buying in expertise from consultants, voluntary sector organisations and other advisors (with a cap on the contribution of £350 plus VAT per day) Promotional activities How to apply The process Stage 1 - Completion of an ‘Expression of Interest’ form which will be used to identify which applications offer the best fit to the Leeds North CCG priorities. Those invited to complete a full application form will be asked to give additional information building upon the information supplied, so reducing unnecessary work for applicants. Guidance notes are available to help you. The deadline for Expressions of interest is 12 noon Thursday 3rd November 2016. Stage 2 - Invitations to submit a full application will be sent by Thursday 17th November 2016; the deadline for submitting full applications and supporting information is 12 noon on Thursday 15th December 2016. The independent grants panel will meet to consider full applications in February 2017 with the aim of project activity starting no later than April 2017. Generally monies should be spent within 12 months of being awarded.
