Tournament on November 19, 2011. You are invited along with your spouse, family
members, and friends to join us on this one day event.
Attached are entry from and tournament rules for your notation. Please make your fee
payable to FASAE SoCal and mail it along with the accomplished entry form to the
Tournament Chairperson, Pert Arca, P.E., 500 E. Carson Plaza Dr., Suite 201, Carson,
CA 90746 on or before November 11, 2011, to give enough time for him to check book
averages and input names and averages and handicaps in the computer.

Bowlers arriving late will start in the frame being played ( no blind scores ).
Falsification of average or name will result in disqualification of bowler of team.
Team Captain is responsible for Team Average and Entry Fee.
Any and all disputes will be decided by the Directors whose decision will be final.
Tournament Director reserves the right to qualify or disqualify any bowler or
re-rate any bowler prior to bowling.
Guaranteed cash prizes will change accordingly if less than 16 Teams enter in this
tournament.
Only 16 lanes are available. Please reserve your team early. First come, first serve basis.
