Parents of High School Athletes, The KHSAA has adopted new physical forms for the 2015-2016 school year. There are separate Parental Consent forms for middle school athletics and high school athletics. GE04 – is the parent permission form for high school level play (freshman, JV, varsity), and is signed by both the student and the parents. Any middle school student playing up at the high school level will be required to turn in a GEO4 along with the new physical form (PPEF). It is very important that any NEW physical for your child must be completed on this form. This new physical form (PPEF) contains a section that is to be completed by the family with health history information, and on the back side, the new physician clearance form. This standard form replaces all prior versions of the physical form. The link below contains both forms (GE04 & PPEF) for students participating at the HIGH SCHOOL LEVEL (freshman, jv, varsity). Students and parents may also pick up a copy of the new form in the DCHS Athletic Office.
