Boys & Girls Club of Newburgh Hudson Valley Panthers 2017 AAU Basketball Program Parent-Player-Coach Contract ACKNOWLEDGEMENT Boys & Girls Club of Newburgh has selected your child, _____________________________ (Player Name) to participate in the 2017 Hudson Valley Panthers AAU Basketball Program. PARENTS ROLE Parents play an important role in the development of a successful basketball player. Parents MUST be a positive influence on the basketball experience and it is imperative that parents speak and act in a positive and acceptable manner throughout the season including games, practices, and tournaments. Coaches must be treated with respect and any questions or concerns regarding the operation of the AAU program should be addressed according to the following guidelines. Parents should call the coach and speak over the phone or set up an appointment to meet with the coach to address concerns regarding the basketball program. It is not appropriate to do this immediately preceding or after a game. Parents if not satisfied following meeting with their sons’ coach may request a phone conversation or meeting with the Program Director to discuss concerns regarding the basketball program. If there is no resolution to the above, the family reserves the right to withdraw the player from the basketball program. Parents must refrain from criticizing or making any negative comments towards their son, teammates, coaches, opponents, officials or other parents throughout the year. PLAYER & PARENT AGREEMENT When our PLAYERS compete, each player is a representative of the Boys & Girls Club of Newburgh and must follow the rules and guidelines listed in this document. As such, players agree to: (1) Follow the General Rules and Guidelines (2) Consistently exhibits good sportsmanship (3) Consistently displays positive behavior. Failure to follow these guidelines will result in immediate suspension from the program and the player’s participation in our program may be terminated. PARENTS must be positive and supportive of players, coaches and opponents. As such, parents agree to: (1) follow the Parents Role: (2) exhibit good sportsmanship: (3) refrain from disorderly conduct and make negative comments about the players and/or coaching staff: or (4) refrain from discussing issues regarding playing time just prior to, during or immediately following games. Failure to follow these guidelines will result in immediate suspension of playing and your child’s participation in our program may be terminated. PLAYER GENERAL RULES OF CONDUCT AS A MEMBER OF BOYS & GIRLS CLUB HUDSON VALLEY PANTHERS AAU BASKETBALL PROGRAM, I WILL ABIDE BY THE FOLLOWING GUIDELINES: (1) I will pledge to make sure that family and academics are my top two priorities (2) I will treat the coaching staff respectfully at ALL TIMES (3) I will play the game of basketball without questioning coaching decisions in regard to who is starting games, substitution patterns and playing time (4) I will be courteous and respectful to my teammates and their parents(5) I will be respectful in public and display good character as I represent the Boys & Girls Club of Newburgh AAU Basketball program (6) I will not participate in any horseplay which may injure my teammates, myself or any other people associated with the Boys & Girls Club of Newburgh AAU Basketball program (7) I will be respectful of gymnasiums and understand that it is a privilege to play in the facilities provided. I will be responsible for my own actions (8) I will be positive and play hard regardless of the score or situation (9) I will follow these RULES OF CONDUCT and I understand that if I fail to comply with these rules, I may be removed from the AAU program (10) I make a pledge that when I step on the court, I’ll be focused on playing basketball and supporting my coaches and teammates. MEDIA RELEASE I agree to allow my child __________________________________to be photographed and/or video/audio-taped by the Boys & Girls Club of Newburgh staff or their designee and for those photos and/or videos to be used for the advertising and promotion of Boys & Girls Club of Newburgh programs. I hereby forever release and discharge the Boys & Girls Club of Newburgh from any and all claims, actions demands arising out of or in connection with the use of said photographs or videos, including, without limitation, any and all claims for invasion of privacy and libel. This release shall inure to the benefit of the assignees, licensees and legal representatives of the Boys & Girls Club of Newburgh. MEDICAL EMERGENCY CONSENT In the event of a medical emergency, first aid will be administered to your child by the Program Director or another qualified individual. Every attempt will be made to reach the parent/guardian first. If further medical treatment is needed, 911 will be called and your child will be taken by ambulance to St. Luke’s Cornwall Hospital. I authorize the Boys & Girls Club of Newburgh to obtain emergency transportation and treatment for my child in the event I am not available. In consideration of the right to participate in AAU Basketball, I hereby assume all risks and will indemnify and hold harmless Boys & Girls Club of Newburgh, its staff, their employees, trustees, volunteers, officers, and members of any and all liability. I attest that I am the legal guardian of the child named above. TRANSPORTATION RELEASE I give permission for my child to be transported in a motor vehicle driven by approved Boys & Girls Club staff. I understand that my child is expected to follow all applicable laws regarding riding in a motor vehicle and is expected to follow the directions provided by the driver and/or other adult volunteers. I understand that participation in the identified event is not a requirement for participation in Boys & Girls Club programs. I have read, understand, and discussed with my child that: 1. They will be traveling in a motor vehicle driven by an adult and they are to wear their safetybelt while traveling; 2. They are expected to respect each other, the vehicles they ride in, and the people they travel with during the trip; 3. Riding in a motor vehicle may result in personal injuries or death from wrecks, collisions or acts by riders, other drivers, or objects; and 4. They are to remain in their seats and not be disruptive to the driver of the vehicle. I recognize that by participating in activities that require transportation, as with any activity involving motor vehicle transportation, my child may risk personal injury or permanent loss. I hereby attest and verify that I have been advised of the potential risks, that I have full knowledge of the risks involved in activities that require transportation, and that I assume any expenses that may be incurred in the event of an accident, illness, or other incapacity, regardless of whether I have authorized such expenses. As a condition for the transportation received, I, for myself, my child, my executors and assigns, further agree to release and forever discharge the Boys & Girls Club of Newburgh and their agents, officers, employees and volunteers from any claim that I might have myself or that I could bring on my child’s behalf with regard to any damages, demands or actions whatsoever, including those based on negligence, in any manner arising out of this transportation. I have read this entire waiver and permission form, fully understand it, and agree to be legally bound by its terms. PAYMENT POLICY Payment schedule is as follows, First payment due March 10th 2017 in the amount of $225 dollars. Second Payment due March 31st 2017 in the amount of $200 dollars. Limited scholarships available upon approval of Program Board of Directors. Financial Aid Forms available at Boys & Girls Club. By signing this document, each player and parent has read and agreed to the policies and procedures set forth in the agreement listed above. Any coach that is on the bench must have a current AAU card and act appropriately. The following guidelines must be followed or the coach may be required to resign from his/her position after a full inquiry and a review from the Athletic Director. (1) Coaches must be in full coaches attire for all games (2) Drinking of alcohol before or during games or disorderly conduct will result in immediate removal from gym and the ELITE Basketball AAU program (3) Physical intimidation of any player will not be tolerated (4) Use of illegal substances will result in immediate termination from program (5) Coaches should keep all player or parent conversations confidential (6) Training by all coaches to enhance their coaching abilities is encouraged.
