Focus on actions that are straightforward to implement, where a recognisable lead person(s) at the faith institution already exists or could be easily identified. Please refer to pages 23-24 in your health-check handbook to see what workshop(s) will also be useful. Also, we encourage you to think critically and creatively to test ideas that are not yet within our current training matrix. Try to suggest between three to five specific actions for each of our five core criteria. Add/print additional pages for each separate action. Du r in g you r f ollow u p visit , ensure that you work with the faith management to find out which actions are most appropriate to respond to the needs of the faith institution. They may also suggest new actions to add to your list. For the actions that they do wish to take forward, complete the remaining sections in collaboration with them. It is vital that each of these columns be completed before signing off on that particular action. Try to make each action as simple as possible, even if it means breaking down more complex actions into constituent parts. I .
