MSDSL Game Sheet Instructions 1. Login to the team GotSoccer account, www.gotsoccer.com. 2. Under the box “Event Registration History”, click the link for the “MSDSL (season) Registration” in the box under Name/Date. 3. Click the tab “Schedule” 4. You will see the season’s sechedule. 5. On the left side of each game, next to the game #, you will see a PDF icon. 6. Click on the pdf icon. 7. It will bring up a download box on the bottom of the screen. 8. You can now open the document and print off your game report. 9. Print (1) game sheet per game. 10. If the player’s jersey number is not indicated, complete the information. 11. Referees verify on the game sheet if the player is present or absent. 12. Pass card (guest) players – write in any passcarded players on the game form with their primary team name. a. You must have available the player’s passcard from his other team at the game. b. Refer to the MSDSL website, www.msdsl.com, under League Information >Rules for the “Club Pass-Card Protocol”. After the game 1. Verify the goals scored on each game sheet. 2. Verify any red or yellow card are marked correctly on game report. 3. The winning team (or home team in the case of a tie) is responsible for calling in the game score within 48 hours of game time. The phone number is provided on the game sheet. 4. BOTH teams are responsible for mailing in their game sheets to the address provided on the game sheet instruction box.
