Want to play Laser Tag with Combined Forces Inc gear and sign this document in
consideration of being given the opportunity to engage in the activities.
I UNDERSTAND THAT:
The Events are physically and mentally intense and may require extreme exertion to play.
The Events can be dangerous if not played in accordance with the safety rules which will be fully explained to the players. The possibility of injury to myself and others exists.
I CONFIRM THAT:
I am fully aware of the risks to myself and others involved in playing Laser Tag and that I will never,
under any circumstances, deliberately break any rules.
I am physically fit and mentally able to take the strain and exertion involved in playing Laser Tag.
I will comply with COMBINED FORCES INC rules and use the equipment as instructed and not so as
to injure or hurt others and will obey all the directions of the COMBINED FORCES INC party leader.

RELEASE:
I hereby release, remise and forever discharge from any claims and liabilities whatsoever without limitation that I might have against COMBINED FORCES INC. and the Owners of the property on which
laser tag is being played and any other player who might injure me howsoever arising, and I make
this release on behalf of myself, my heirs, executors and administrators.
RENTAL AGREEMENT:
I agree to return all equipment at the end of the event to COMBINED FORCES INC. I have read and
fully understand all the terms of this document and confirm that I am not relying on any statements or
representations by any person or entity as an inducement to my fully and voluntarily engaging in Laser Tag assuming the risks and obligations stated above signing this document.
PHOTO AGREEMENT:
I agree that Combined Forces Inc. may use such photographs of me with or without my name and for
any lawful purpose, including for example such purposes as publicity, illustration, advertising, and
web content.
