This application will help determine your eligibility for the Housing Assistance Program – Plaquemines Initiative
(HAP-PI). In order for you to be considered for the homeownership assistance pilot, you must complete the
Personal Profile Application income eligibility verification:

Once the application has been completed to the best of your ability and the documents have been compiled, please
call Paula Pete, of Family Resources New Orleans (FRNO) at (504) 822-8519 to schedule an appointment.
Please note that this program is on a very limited timeframe and first come first serve basis. If you have any
questions or need help completing this application, please contact Ms. Pete for assistance.

The following information is necessary to complete your eligibility verification. Partial information will not be
accepted. When submitting documentation, please make sure you bring copies. FRNO cannot make copies
of your originals or return your documents once they are submitted.

_____ Proof of Income (3 most recent check stubs, Social Security Award Letter, Retirement Pension Letter, or
any other documentation that establishes your current income)
_____ Signed Certification of Zero Income (for any adult 18 and older), living in the household with no income)
_____ Proof of Residency (Driver’s License, Social Security or Pension Award Letter, Utility Bills)
_____ Proof of Assets (savings/checking account, CDs, retirement funds, 401k, etc.)
_____ Recent Bank Statements (from the last two months)
_____ Previous Year’s Tax Returns (2011 / 2012) complete with the 1040 FORMS & W2 FORMS
