Prior to drafting any contract for goods or services, the responsible party or the decedent’s survivor who is handling the funeral arrangements is entitled to receive
a copy of any pre-need agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent.

Our Story
As a teenager, I found an audio tape of my grandmother a few years after she had died. As my family listened to the voice and the laughter of someone
we had loved and lost, we were inspired by an idea we have chased ever since. We became dedicated to capturing the essence of lives and personalities
– recording and preserving these memories with the latest technologies – striving for the dream that some day no one will be forgotten – that each of us
will be able to personally tell our own stories to the future. This is the promise of FOREVER.
This dream unexpectedly led us to funeral homes and cemeteries – where memories have a special resonance and purpose. In 1986, a client of our
biography business died, and his son asked if we would show his father’s biographical documentary at the funeral. As we watched the deceased appear
on a screen at his own funeral – sharing his life stories and professing his loves one last time - we knew we had found the heart of our dream.
In grief and loss, memory is one of the most potent antidotes. During the most unbearable of times and the most inexplicable of tragedies, tangible
memories of love and life can heal and sustain. They remain with us - remnants of life - outliving their source by surviving within us.
You have come to Hollywood Forever to remember someone you have known and loved – or perhaps you have come to decide how you wish to be
remembered. Though death is the cause of our meeting, let us endeavor to celebrate life. Our hope is to help you fill the emptiness of loss by assisting
you in giving expression to the richness and particular beauty of a life that was like none other.
Here, the only rules and traditions are the ones you walked in with. If religion or spirituality brings you solace, comfort, and meaning at this time –
we will work with you to bring your beliefs and your heritage into the heart of the ceremony. All faiths and creeds are welcome – and we have sought
the knowledge and understanding to serve your traditions with truth and authenticity.
If the traditions of the past do not speak to you, then allow us to reinforce the freedom you have to find a new way to honor and express the spirit
and the personality of the one you have lost. We encourage you to leave behind any preconceptions of what a memorial should be. Create your own
path, discover your own rituals of honor and remembrance – be they profound, humorous, sacred, or profane. We’ll be there right beside you – step
by step – trying to anticipate your needs and fulfill what you envision.

While we encourage you to customize your arrangements from the itemized list above, we offer service packages
that include the most frequently selected items as a means of simplifying the process and lowering the costs.
Our service packages reflect discounts to the prices of individual items, so substitutions are not possible, and credits
are not applied if you choose not to make use of any of the included items.
Our service packages are available only when the casket is purchased through our funeral home. If you purchase
the casket elsewhere, please select from our itemized list of services and merchandise to complete the funeral service of your choice. Services and merchandise, whether included in a package or not, may be purchased separately.

This itemized price list offers an overview of the general goods and services offered by Hollywood Funeral Home.
The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you
select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will
explain in writing on the statement we provide describing the funeral goods and services you selected. Likewise, items or services you may ask us to purchase for you
(such as cemetery fees, newspaper notices, and other cash advance items), will be shown on your invoice or statement describing the funeral goods and services

This basic fee includes the services of the funeral home staff’s professional expertise in assisting with all the requirements of a funeral arrangement.
The funeral staff is made available to respond to the initial call for service. The funeral home staff is made available for the arrangement
conference, in order to document the requested funeral rights and to then coordinate the details with clergy, cemetery, crematory, florists, escorts,
musicians, Health Department, common carrier, and any relevant publications for the placement of obituary notices, as determined by a family
and/or other parties responsible for special services or disposition. This fee includes a proportionate share of the established overhead costs, city,
county, state and federal indirect taxes beyond our control.

This service provides professional care and preparation of the deceased. Hollywood Funeral Home’s highly trained and exceptionally qualified
staff are made available twenty four hours a day to cleanse, sanitize, embalm and prepare for viewing and/or disposition of the deceased body
using modern, scientific methods to safely ensure public health, proper discharge of infectious materials and contaminants. All procedures are in
strict compliance with OSHA guidelines. Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you
select certain funeral arrangements such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does
not require you to pay for it, such as direct cremation or immediate burial.

Cremation Services
From cultural tradition to personal preference, there are many different reasons to choose cremation. Our cremation options range from the simplicity of direct cremation with no ceremony to a traditional funeral service followed by the cremation.
Please remember that service options for cremation are as plentiful as those for any other type of funeral service. You may decide on flowers, service
folders, musical selections, or a dove release. You may choose to have clergy officiate or to have family and friends speak. Whatever your wishes, your
counselor will assist you in creating a funeral or memorial service to best honor and remember your loved one.
Because our options include various types of services, no credits for deletions or substitutions are possible.
PLEASE NOTE:
Cremation containers, caskets, urns, and outer burial containers are not included in our cremation packages. Instead, they are offered on separate
price lists, which will be provided by your funeral counselor. In addition to our Casket, Urn, and Outer Burial Container Price Lists, please review
our itemized General Price List for a more detailed account of merchandise and services we offer.
Unless otherwise specified, all cremations will take place at Hollywood Forever Crematory, on the grounds of Hollywood Forever Cemetery.

If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide range from fiberboard (heavy cardboard) to hardwood (walnut).
