Adult Community Services Home First Care Certificate Team Leaders Marking Checklist for Observation 1 Understand your role – standard 1 Clear understanding of customers support plan and meeting customers’ needs in a person centred way (C.C.1.1.c; 1.2d refer). Include Enablement Demonstrate ways of working with others (C.C 1.4c, 1.4d refers) Note: 1.1c Demonstrate that they are working in accordance with the agreed ways of working with their employer. 1.2d Demonstrate how to access full and up to date details of agreed ways of working that are relevant to their role. Note: 1.4c requires the HFP to provide performance evidence however the opportunity to do this may not arise during the period of induction. Other evidence to show that the HFP would be able to do this if real work evidence is not available is permissible. 1.4 c Demonstrate ways of working that can improve partnership working 1.4d Demonstrate where and when to access support and advice on partnership working and resolving conflict. (role play may be used if not observed in probation period) Personal Development - standard 2 Develop Knowledge Skills and Understanding Note: to be part of your 1:1 discussions so as part of probationary meetings whilst doing care certificate Agree a personal development plan Contribute to drawing up their own personal development plan Demonstrate how to measure their own knowledge, performance & understanding against relevant standards. Demonstrate how to record progress in relation to their personal development. 2 Duty of care - standard 3 Deal with comments and complaints (C.C 3.3a, 3.5d) Note: Role play is allowed 3.3a Demonstrate how to respond to comments and complaints in line with legislation and agreed ways of working. 3.5d Demonstrate how and when to access support and advice about resolving conflicts. Equality & Diversity - standard 4 Work in an inclusive way, respect for customer privacy, dignity, independence, beliefs, culture, values, preferences, choices and self-esteem (C.C.4.2B) Note: 4.2B Demonstrate interaction with individuals that respects their beliefs culture, values and preferences. 3 Work in a Person Centred Way – standard 5 Due regard for health/safety/risk management – e.g. heating, lighting, noise, temperature, unpleasant odours etc. Ensure that where individuals have restricted movement that they are comfortable and appropriate steps have been taken where there is pain or discomfort to reduce it. Make others aware of concerns, actions and support these individuals. Support the individual to maintain their identity and self-esteem. (C.C No. 5.3a, 5.3B, 5.4a, 5.4B,5.4c, 5.5a, 5.5B, 5.5c & 5.5d, 5.6B, 5.6c & 5.6d, 5.7a refers) Note: Role play is allowed if not observed in probation period. 5.3a Demonstrate awareness of the appropriate steps to remove or minimise environmental factors that are causing discomfort or distress include: - lighting, noise, temperature, unpleasant odours. 5.3B Report concerns to family or team leader. 5.4B Advice team leader of steps taken to reduce discomfort. 5.4c Raise concerns with others at a team meeting or to OT. (Role play can be used for 5.4) 5.5a Check with individuals with restricted movement that they are comfortable. 5.5B Recognise signs that an individual is in pain or discomfort, verbal communication, nonverbal communication, changes in behaviour. 5.5c Take appropriate action to remove pain or discomfort. Re-position; give pain killers if appropriate, ensure equipment is working properly, and in the right position. Report what has taken place to a team leader. 5.5d Remove any environmental factors causing discomfort or pain. Such as soiled pads or bedding, noise or temperature. 5.6B Demonstrate that their own attitudes and behaviours promote emotional and spiritual wellbeing. 5.6c Support and encourage individuals own sense of identity and self-esteem. 5.6d Report any concerns about an individual’s spiritual and emotional well-being to a team leader. 5.7a Demonstrate that their actions promote person centred values including Individuality, Independence, Privacy Partnership Choice, Dignity, Respect rights 4 Communication – Standard 6 Demonstrated communication skills (verbal & nonverbal) (C.C. Nos. 6.5a, 6.6a & 6.6B refer) Note: 6.5a verbal: - tone, volume. Non-verbal:- position/proximity, eye contact, body language, touch, signs, symbols & pictures, writing. Objects of reference, human & technical aids. Communication: - face to face, by telephone or text, by written reports. 6.6a Ensure any communication aids are clean, work properly, in good repair. Report any concerns about the communication aids to the appropriate person: - team leader, member of the family. 6.6B Report any concerns about the communication/aid technology to the appropriate person. (This may be done in role play if not observed in probation period) 5 Privacy & Dignity – Standard 7 Support the individual in active participation in their own care including administration of medications. (C.C 7.2a, 7.2B. 7.4a, 7.4B 7.6a refers) Note: 7.2a Demonstrate that their actions maintain the privacy of the individual. Using appropriate volume to discuss care & support of the individual and in a place where others cannot over hear (team meeting may be appropriate) 7.2B Demonstrate that the privacy and dignity of the individual is maintained at all times being in line with the person’s individual needs and preferences. This could include Making sure doors or curtains are in the correct position Getting permission before entering someone’s personal space Knocking before entering a room Ensuring any clothing are positioned correctly The individual is positioned appropriately and is not exposing any part of their body they would not want others to be able to see 7.2d Report any concerns they have to a team leader. (This can be in role play if this does not arise in probation period) 7.4a Demonstrate how to support individuals to make informed choices. 7.4B Ensure any risk assessment processes are used to support the right of individuals to make their own decisions. 7.6a Demonstrate that they can support the active participation of individuals in their own care. 6 Food & Nutrition – Standard 8 Practically applied support with nutrition – food and drink (C.C. Nos. 8.2 a.b.c.d.e.; & 8.3 a.b.d.e. refer) Note: 8.2a Check that food & drinks are within easy reach of those that have restricted movement or mobility. 8.2B Check that drinks are refreshed on a regular basis for those with restricted movement or mobility. 8.2c Check that individuals are offered drinks or offered to assist to make drinks for themselves in accordance to their care plan. 8.2d Support & encourage individuals to drink in accordance with their care plan. 8.2e/8.3e Know how to report any concerns and know the relevant person to report this to. (May be in role play if does not arise in probation period) 8.3a Check that any nutritional products that are required in care plan are within reach to the individual with restricted movement or mobility. 8.3B Check food is provided at the correct temperature and made in accordance to the care plan. 8.3c Check that the appropriate utensils are available to the individual to enable them to eat as independently as possible. 8.3d Support and encourage individuals to eat in accordance with their care plan 7 Safeguarding Adults – Standard 10 (C.C. No 10.1j refers) Note: 10.1j Demonstrate the importance of ensuring individuals are treated with dignity and respect when providing health & care services. Health & Safety – Standard 13 Safely handled hazardous substances, Demonstrate how to move and assist people safely. (C.C. No. 13.3c &13.6B refers) Note: 13.3c Demonstrate how to move and assist people safely maintaining individual’s dignity and in line with legislation and policies in agreed ways of working. Handling Information – Standard 14 (c.c. 14.1c refers) Note: Demonstrate how to keep records that are up to date, accurate and legible. Infection prevention and control – Standard 15 (C.C.
