TEAM REGISTRATION CHECKLIST
All teams registering for the Cincinnati West Soccerfest must present the proper materials, fully
completed, in order to participate. We do not register players or teams at the fields. All registration is
handled by MAIL (use this option if you can) or on registration night Friday May 6th from 6:00pm-8pm
at Courtyard Marriott North at Union Centre 6520 Muhlhauser Road West Chester, Ohio 45069.
Please see that you have everything on this list fully completed prior to sending in the mail or going to
the registration night. Note we are no longer requiring copies of player/coach cards or medical
release forms. Player and coach cards need to be brought to the fields for check in before each
game.

List of forms ALL teams need to provide:

A few things you should do to make the process go smoothly:
• Please cross out all coaches and player on the roster who will NOT be at the tournament.
Any coach or player who is not crossed off roster will need to sign the team liability form
• All guest players need to sign team liability form as well
We strongly encourage mail-in registration. (MUST BE MAILED BY FRIDAY April 28, 2017)
