Minor’s Form for Youth Participation
By our signatures below, we acknowledge that we are aware of, appreciate the character of, and
voluntarily assume the risks involved in participating in Camden County 4-H’s Next Big Thing
Talent Show.
1. Waive any claim or cause of action against and release from liability Camden County
4-H, Camden County Board of Freeholders, and Miller Farms, its officers, employees,
and agents for any liability for injuries to person or property resulting from participation
in the activity listed above;
2. Agree to indemnify and hold harmless Camden County 4-H, Camden County Board of
Freeholders, and Miller Farms, its officers, employees, and agents for any claims, causes
of action, or liability to any other person arising from participation in the activity listed
above.
3. Consent to receive any medical treatment deemed advisable during participation in the
activity listed above; and
4. Acknowledge that we are signing below as a minor child and as the parent or legal
guardian of the minor child named below.
5. Agree to allow Camden County 4-H to use photographs and likenesses taken during the
performance in promotional materials.

Camden County 4-H’s Next Big Thing
Talent Show
Official Rules and Information
1. Camden County 4-H’s Next Big Thing Talent Show is open to all AMATEUR
entertainers between the ages of 5 and 18. Professional (musician or performers whose
principle source of income) are not permitted to enter. Membership in 4-H is not
required.
2. Acts may be any type of talent, musical instrument, song, dance, magic, etc. Winning
acts will be selected for group acts and individual acts.
3. Entry forms need to be received no later than Monday, July 30, 2012. All entries
received after this date will be ineligible. Act timeslots will be filled once entry form is
received.
4. The time and date of Camden County’s Next Big Thing Talent Show is Saturday, August
4, 2012 at 5:00 pm. Your time slot will be assigned to you, and you will be informed of
it once you arrive at the show.
5. All contestants must arrive at the show by 4:30 pm.
6. Contestants may participate in both the group and individual categories, but must pay
said entry fees for each category.
7. Vocalists may use LIVE OR TAPED MUSIC WITH NO VOCAL BACKUP. All
contestants are required to use the sound system provided at the show.
8. All acts will have no more than 5 minutes to perform. The judges will maintain timings
for all of the acts. Acts that violate this time limit will be disqualified.
9. This is a family talent show. The judges and Talent Show organizer reserves the right to
disqualify any act that does not display appropriate material.
10. Camden County 4-H’s Next Big Thing Talent Show will be held on the Camden County
Showmobile (portable stage). There are no changing facilities at the show.
11. All decisions of the judges are final.
The 4-H Youth Development Program is part of Rutgers Cooperative Extension. 4-H educational programs are offered to all
youth, grades K-13, on an age appropriate basis, without regard to race, religion, color, national origin, gender, sexual
