Practices have brought to our attention that requests are still being received to share the care of NHSE commissioned drugs. These currently include: 1. Drugs for renal dialysis and nephrology outpatients: Darbepoetin, Epoetin alfa / beta/theta/zeta, Cinacalcet, Lanthanum, Sevelamer etc. 2. Immunosuppressants post transplant in patients with a complete care package: Tacrolimus tabs, Mycophenolate mofetil, Ciclosporin, Mycophenolic acid, Sirolimus 3. Nebulisation/inhalation antibacterials and mucolytics in cystic fibrosis: Tobramycin, Colistimethate sodium, Aztreonam lysine, Dornase alfa 4. Drugs for viral hepatitis B: Adefovir, Entecavir, Lamivudine, Motavizumab, Telbivudine, Tenofovir The Medicines Management Team (MMT) is working to establish mechanisms of recharging NHS E for existing prescribing in primary care. Action for practices Practices are asked to note the indications and associated drugs above. New requests to share care in the stated indications should not be upheld as these are specialist commissioned drugs. However, please continue to prescribe for current patients until you receive a specific letter asking you to stop. Feel free to contact the MMT if you have any queries. Practices are asked to note the drugs for commissioning by NHS England. These are not for new prescribing in primary care INSULIN PASSPORTS—REMINDER Practices are reminded that patients on insulin therapy should be encouraged to carry their insulin passports at all times and ensure they are completed with accurate information. This is in line with the National Patient Safety Agency (NPSA) alert ‘The adult patient’s passport to safer use of insulin’ 2011 (link) which requires that all adult patients on insulin are issued with an insulin passport and a patient information leaflet/booklet. The insulin passport is a credit card sized folded paper which contains information on the insulin brand and presentation/device in use. It is not a daily dosing record. Action for practices At initiation of insulin, provide patients with an insulin passport and patient information leaflet. As part of the annual diabetic review, check that patients on insulin have an insulin passport and encourage them to carry it at all times especially when visiting healthcare professionals. Ensure the passport is completed with the most accurate information. Where further supplies are required, orders can be placed with 3M via the usual processes.
