reference / confirmation from the superior about your daily duties, specialty, narrower
field of work, etc.).
Note: Assessors are obliged to up date their personal records by fulfilling and submitting an OB
03-09 form Application for assessor/expert in the accreditation system once a year and if there be
any changes to submit adequate evidences (for example training certificate, new employment,
etc…). If there be no changes assessors must inform the Personnel evaluation Committee in
written (by e-mail), too.
8. STATEMENT OF THE CANDIDATE FOR ASSESSOR/ASSESSOR/EXPERT
I confirm the accuracy of data provided and I commit to regularly submit all modifications
referring to provided data, primarily to data concerning qualifications, and its modifications
(additional qualifications, or termination of existing qualifications)
I declare that according to IARM’s document stating the requirements to be satisfied by the
assessors and experts in the IARMS’s accreditation system, I will duly inform you about all
modifications made, in written, as to enable timely and consistent update of the latest personal
data and their utilization.
According to the Law on Protection of Personal Data (Official Gazette of R.M. No. 7/2005,
103/2008 and 124/2010) I authorise usage and publication of my personal data for IARM’s needs
as well as their release to foreign Accreditation Bodies on demand.
