Adult Mental Health Service FACE Team For those who frequently attend at emergency services ^ ãÉãÄÉê çÑ `~ãÄêáÇÖÉ råáîÉêëáíó eÉ~äíÜ m~êíåÉêë This leaflet explains what the FACE team is all about The Team..... FACE stands for Frequent Attenders, Care Enhanced. It is a new team which is being piloted for 12 months from April 2012 in this area and you have been referred as there are concerns about the frequency with which you are accessing emergency services. FACE is made up of a group of staff from many different backgrounds. They are as follows: • • • • • • • Team Leader/ Project Manager Administrator Community Psychiatric Nurse Substance Misuse Specialist Senior Social Worker Peer Worker Research Assistant The team will primarily be based at Nightingale Court on the Ida Darwin site at Fulbourn How we are different from other services..... • You will spend up to 8 weeks with the FACE team • During this time we will assess your needs and support you in addressing them • When you are assessed we will take into condsideration all of the different organisations already involved in your care • We will work closely with you to support you in accessing services and other organisations that you feel may suit you • We will work alongside the services and organisations involved in your care to develop a plan that will enable you going forward • We will respect your individual preferences and will offer you as much choice as possible where your care is concerned What happens next..... When we begin to work with you, information from those services that provide support to you will (with your permission) • provide FACE with information so that we can provide the best possible care for you. • be involved in the enhanced care planning You will be allocated a worker who will be your main point of contact within the team whilst you are with us. Your carers..... Wherever possible, and with your consent, your relatives and/or carer/ s should be involved in the development and implementation of your care as outlined and chosen by you on your Care Plan. Information and support for your carer/s can be accessed through your Care Coordinator. Carers should be offered an assessment of their needs. The FACE team aims..... • to provide a good quality and flexible service which improves care and support for you if you frequently attend emergency services • to help increase your ability to connect with the broadest possible range of services that suit you by being adaptable to your needs • to encourage you to develop and sustain a support network and build on relationships that may help to prevent future attendance at emergency services • to provide you with the opportunity to become involved in organisations and/or activities that may result in an overall experience of improved quality of life over a period of time The future..... As part of the process your FACE team worker will support you to develop a plan of care which will identify which services are most suited to your individual needs. Following your discharge from the FACE team, your plan will enable you to form the basis of your continuing development using the services outlined in the plan. Contact details.....
