The surgery has a new email address that can be
used to request any items that are not on a
repeat prescription.

Please make sure you include your name and date of
birth when making a request.
For ordering items on your repeat prescription please
register for the Vision Online Services – forms can be
picked up at reception or downloaded from our
website.
