WHO IS INVOLVED?
The Home Treatment Team is a
multi-disciplinary
team
of
professionals
including
Occupational Therapists, Social
Workers, Nurses and Support
Workers.
We have medical cover on a daily
basis and can refer clients to
psychological
services
as
appropriate.
We want to work closely with the
people who care for you so that
they can help you the best way
possible and so that they too are
supported.
We are available 7 days a week,
365 days a year from 8.30am to
9pm. Outside these times, clients
we are working with have direct
access to advice/support from the
ward who will be aware that the
client is under the care of the
Home Treatment Team.


WHAT IS A CRISIS?
A crisis is when you feel a situation
is out of control and you need
urgent help. Mental health problems
can sometimes cause a person to
behave in a way which is unsafe for
themselves or others. Also, there
can be a build up of stress for the
person and those around them and
help may be needed to deal with
this too.
The Home Treatment Team will
work with you, your carers and
those involved with delivering other
aspects of your care.
It offers you a short term, intensive
treatment programme of practical
tasks, support and advice. The aim
is to promote your wellbeing,
particularly when you or your carer’s
health or circumstances are
changing.

absolutely necessary

HOW MIGHT THE
WORK FOR YOU?

We will arrange to meet you and
your carers to discuss your
current circumstances

Once specific needs have been
identified we will work with you
and your carers to create a
treatment plan

We will meet with you at regular
mutually agreed times, trying to
help meet these needs and
overcome the crisis

Prior to discharge we will sit
down with you and your carer
and
formulate
a
relapse
prevention plan so all parties
can ‘learn from the crisis’

We will strive to ensure a
smooth handover of your care to
appropriate
services
i.e.
CMHT/GP or we may direct you
to other appropriate voluntary
organisations
