HEALTH DISCLOSURE DOCUMENT

In this health course, students will learn about mental, physical and social well-being. Students will also
have the opportunity to establish and achieve goals in these areas, which will contribute to a healthy
lifestyle and lifelong wellness.

The curriculum for this course has been established by the Utah State Office of Education and can be viewed

MATERIALS NEEDED:

FOLDER: Students are required to have a folder specifically for Health. Organization is key in
succeeding in this class. Packets, Assignments, Quizzes, Tests, etc. will be stored in the health
folder.
PEN/PENCIL AND PAPER: Each student must bring a pen/pencil and paper to class daily.
GRADING PROCEDURES:
Students will be graded on:
Attendance: Each student will be given 150 participation points to begin with. Students can lose these
points from not participation in class… which means student needs to be in class to keep them.
Assignments/Projects: All assignments will receive full credit on the day it is due. An assignment turned in
late during the current unit will be docked 20%. If an assignment is turned in after the Unit Test the
assignment will be docked 50%.
Quizzes: Quizzes will be given periodically to check student understanding of the topic.
Tests: Each unit will include a test on the content. There will be a class review session the period before
the test is given and each student will be given a test review sheet from which to study. If a student does
not do as well as he or she would have liked, he or she is free to retake the test only if the unit
work/packet is complete and the unit review sheet has been completed correctly.
Academic integrity-Students caught cheating will receive a “0” on that assignment. Cheating includes copying another
student’s work, plagiarizing, and using unapproved materials during a test. In addition, a student log entry
will be submitted documenting the incident and your parent/guardian will be contacted. Remember, it is
your responsibility to make yourself appear above suspicion.
**Only 3 assignments per term can be turned in late. Each term students will receive 3 late passes from me.
When you want to turn in an assignment late, staple one of the late passes to the assignment. All late work
going on the term grade is due 1 week before the end of the term. Remember, life is easier when you
stay on top of your due dates.
Students who are excused from school will receive 1 day for each day they are out to make up their
work. Please know that if you are out, it is your responsibility to see me and find out what you missed.
How am I doing in your class?
I expect you to access your class information (grades and attendance) weekly via Power School. Parents
can set up Power School to e-mail them information on a regular basis, and I encourage them to do
so. Please allow me 3-5 days to enter scores after their due date. I update grades weekly.
Letter grades will be distributed according to the following grade scale:
94 - 100 = A
80 - 82 = B-
67 - 69 = D+
90 - 93 = A-
77 - 79 = C+
63 - 66 = D
87 - 89 = B+
73 - 76 = C
60 – 62 = D-
83 - 86 = B
70 - 72 = C-
0 – 59 = F
CLASS MANAGEMENT
A well organized classroom promotes learning, a disorganized one prevents it.
Students Will:

Put cell phones, music devices, or games away during instructional time.
Not talk when the teacher or a student is addressing the class.
Follow directions the first time.
Be prepared for class every day.
Use appropriate language and tone of voice.
Be in assigned seat when the bell rings.
Have no food, gum, or sugary drinks in class. Water in a bottle is acceptable.
Discipline Procedures:
• Warning. Reminder of the rules.
• Seat change/Time Out seat
• Phone call or email to parent
• Severe infraction: Sent to the office.
